Radiological catheter drainage of abscesses in children

Abstract: Imaging-guided catheter drainage of fluid collections in children is safe and effective. Newer approaches, better imaging, and improved techniques have resulted in improved cure rates. The basic principles, common indications, various techniques, and the possible complications of these procedures, with special reference to the pediatric population, are reviewed in this article.

“…These include standardizing US scanning technique, 23 establishing clinical pathways that combine US with laboratory data to improve diagnostic accuracy, 24 and developing training and credentialing programs to enhance the US capabilities of existing personnel 25 . With respect to justifying use of US in this cohort, the efficacy of US in guiding percutaneous drainage of intra-abdominal abscesses in children has been reported to be between 85% and 90%, comparable to rates reported for CT-guided drainage 26–28 . As such, the Society of Interventional Radiology recommends for the percutaneous drainage of abscesses in children that US should be used whenever possible to spare potential radiation exposure, including substituting US for CT where feasible 29 .…”

[26][27][28] of abscesses in children that US should be used whenever possible to spare potential radiation exposure, including substituting US for CT where feasible. 29 The number of patients in this study successfully diagnosed (and percutaneously drained) with US alone represents a proof of concept of this approach, and it is noteworthy that the 2 hospitals with the highest CT-associated DER were also positive outliers in use of US as the first-line imaging modality.…”

“…Imaging-guided percutaneous aspiration or drainage using sonographic guidance was first reported in 1974 in adults and now has been applied in children [6][7][8][9][10][11][12]. Tan et al [22] reported a series of six cases, three of whom were solitary abscesses that were treated by open drainage in two and antibiotics alone in one with good results.…”
