Radioimmunotherapy-Based Conditioning Regimen Prior to Autologous Stem Cell Transplantation in Non-Hodgkin Lymphoma

Abstract: Non-Hodgkin lymphoma (NHL) is the most common hematologic malignancy and the sixth cause of death from cancer in the USA. Autologous stem cell transplantation (ASCT) is a potentially curative therapeutic option for many NHL patients. Choosing the most effective conditioning regimen prior to ASCT can lead to longer survival in these patients, and, as in many cases of high risk NHL, the only potentially curative option is stem cell transplantation. Radioimmunotherapy (RIT) is based on using radiolabeled monoclon… Show more

“…The reasons for the lower effect on solid tumors may include poor tissue perfusion of solid tumors, resulting in poor anti-tumor efficacy, and the tendency of tumor cells to develop radiotherapy resistance 117 . In addition, IV RIT exerts hematologic toxicity due to its long blood circulation time and the possibility of inducing hematologic malignancies 118 , 119 . It should also be emphasized that RIT relies on highly specific targeted antibodies.…”

“…Additions to standard conditioning regimens are undergoing evaluation to deepen consolidation responses and reduce the risk of relapse after autoHCT with a focus on cellular and immune-based therapies. Radioimmunotherapies, using an antibody labeled radionuclide to target the radiation's effects, like 131 Iodine-tositumumab and 90 Yttrium-ibritumomab tiuxetan have been studied but are not commonly used due to the complexity of administration, delayed engraftment, and prolonged cytopenias (74). An ongoing phase 2 trial evaluating a novel anti-CD25 radioimmunotherapy ( 90 Yttriumbasiliximab) combined with BEAM conditioning (aTAC-BEAM) is ongoing (NCT04871607).…”
