Chronic venous insufficiency is a common problem leading to varicose veins of lower limbs which was traditionally being treated with ligation and stripping. Endovenous ablation is an attractive alternative minimally invasive technique to treat such patients. We analysed the results of 100 consecutive patients treated with Radiofrequency based endovenous ablation using Closure Fast Catheter. Retrospective analysis of 100 consecutive patients with chronic venous insufficiency treated at our institution was done. All patients were classified pre and post procedure based on clinical, etiologic, anatomic and pathophyslogical classification, Venous clinical severity and Venous disablitity scores alongwith color doppler examination with follow up done at 2 weeks, 3 months and one year for any disease recurrence or residual symptoms. 97 of the 100 patients treated at 3 months and at 1 year showed good reduction of venous clinical severity and Venous disability scores to 0. Doppler showed complete closure of saphenofemoral junction and long saphenous vein at 2 weeks with no recanalisation at one year. No thrombus formation was observed. Minor complications like ecchymosis was seen in 9 patients and thrombophlebitis in 4 patients. Endovenous ablation using radiofrequency with Closure Fast technique showed improved short and long term results in patients with venous insufficiency with 100% closure seen on doppler studies along with lesser complications and no thrombus formation due shortened procedure time and improved catheter design.

На рубеже столетий во флебологии произошли значительные изменения. Это связано с двумя клю-чевыми событиями -широким внедрением точной и детальной визуализации венозной системы конеч-ностей с помощью ультразвукового ангиосканиро-вания и появлением миниинвазивных эндоваску- . Частота миниинвазивных флебэктомий в «стационаре одного дня» составляет 340 в год. Не выявлено снижения количества операций при деком-пенсированных и осложненных случаях ВБНК. Традиционная флебэктомия, выполненная в круглосуточном стационаре, сопровождается в среднем 40 днями нетрудоспособности, что в масштабах города составляет 72 600 дней нетрудоспособ-ности в год. «Стационар одного дня» эффективнее, тем не менее после операций в нем число дней нетрудоспособности составляет 7400 в год. Эндовазальные амбулаторные вмешательства за три последних года сэкономили для города 304,4 тыс. дней нетрудоспособности. Заключение. Использование возможностей амбулаторных медицинских центров, специ-ализирующихся на оказании флебологической помощи, может существенно повысить эффективность лечения варикозной болезни в масштабах крупного города. Ключевые слова: варикозная болезнь, стационарная флебохирургия, амбулаторные эндовазальные вмешательства.Aim -the objective of the present study was to elucidate the tendencies in the development of the surgical treatment of primary varicose veins of the lower extremities (PVVLE) in the residents of a large industrial city during the past 5 years. Material and methods. We undertook the analysis of the statistical data on the patients presenting with PVVLE and operated in a round-the-clock clinic of Ekaternburg during the period from 2009 to 2013. In addition, similar data concerning the patients with the same pathology treated at an outpatient basis in the periods between 2005 and 2007 and from 2011 to 2013 were available for analysis. Results. The total number of conventional phlebectomies performed annually varied from 1627 to 1957 (mean 1815±165). The number of open interventions for the treatment of primary varicose veins showed no tendency toward a decrease for the last 5 years. One third of the patients presenting with PVVLE have undergone the surgical treatment based at the departments of general surgery. The number of sclerotherapeutic (CT) procedures increased and reached11,777 (or 80.4% of all ambulatory interventions performed in the medical centers (MC) in 2013. The number of endovasal thermal obliterations in 2013 amounted to 2585; 74.6% of them were laser-assisted procedures. The number of minimally invasive phlebectomies performed based at one-day clinics was 340 per year. The number of surgical interventions for the treatment of decompensated or complicated cases of primary varicose veins did not decrease. Traditional phlebectomy performed in a round-the-clock clinic was followed by a 40 day-long (on the average) period of incapacity for work. On the city-wide scale, the number of inability-to-work days amounted to 72,600 per year. The one-day clinics were more efficacious in this respect even though ...
