Radioactive seed localization in breast cancer treatment

Abstract: In the present study of more than 1200 patients and 7 years of experience, RSL was shown to facilitate breast- and axilla-conserving surgery in a diverse patient population. There was a significant reduction in resection volume while maintaining low positive resection margin rates after BCS.

“…Catharina Hospital had one of the lowest positive margin rates, 4.6% (15/325) with RSL, but there was no corresponding WL group (Van Riet et al, 2010). The Netherlands Cancer Institute (Janssen, et al, 2016) reported positive margin rates with RSL as 9.1% for DCIS and 9.7% for invasive cancer, but likewise there was not a comparison WL group. A Canadian multicenter randomized trial also favorably compared RSL to WL, with positive margin rates of 10.5% for the 152 RSL patients and 11.8% for the 153 WL patients (Lovrics, et al, 2011).…”

“…However, TCC is much less prone to dislocate compared to other markers, and in our series no lesion missing occurred . Recently, promising results emerged from evaluation of RSL and magnetic seeds, since they conjugate the advantage of an intraoperative guide such as ROLL, with the high accuracy of localization and easier logistics associated to TCC …”

“…With the increasing quality of mammographic imaging and the introduction of wide-ranging screening campaigns for breast cancer, non-palpable tumors are detected earlier and to a higher degree [1][2][3]. Consequently, breast conserving surgery (lumpectomy) has become an attractive option compared to whole-breast surgery (mastectomy) [3].…”

“…This can be explained by the excision procedure of non-palpable tumors, which is highly dependent on accurate target delineation (pre-localization) needed for surgical guidance [1,2,5]. There is a clear need for improved preoperative image-guided localization in order to improve resection and recurrence rates [1,5].…”
