Radical surgical treatment of neuroendocrine tumors of the appendix in children – a Polish multicenter study

Abstract: IntroductionThe aim of study was to examine management of pediatric appendiceal neuroendocrine tumors (ANET) in Poland.Material and methodsRecords of 27 patients with ANET diagnosed incidentally after appendectomy in last decade.ResultsWell-differentiated NET G1/G2 was diagnosed in 25 and well-differentiated neuroendocrine carcinoma G3 in 2 patients. Extended surgery was performed primary in one instance and secondary in 10 patients (right hemicolectomy in 9, ileocecal resection in one) without adjuvant chemot… Show more

“…In addition, a recent meta-analysis has also observed that no relapse or tumour-related death occurred in all published cases of paediatric appendiceal NET [ 28 ]. As far as we know, the only case of relapsed appendiceal NET, published in 2021, has size as only risk factor [ 29 ].…”

“…3,28,36 Ileocaecal resection either as first line surgery or as completion surgery has also been applied in some patients, 11,36 mainly in paediatric populations. 37,38 Although the indication for ileocaecal resection instead of RHC in aNET remains unclear, resection of the ileocaecal artery at its origin from the superior mesenteric artery is recommended. 36 Factors to consider when deciding on one of the two types of intestinal resection, are the LN yield, short-term surgical morbidity, and long-term outcome regarding HRQoL.…”

“…Of the multitude of studies on pediatric appendiceal NENs, most report no observed recurrences in their patient populations [ 109 ]. One study out of Poland has documented a recurrence after surgical resection (which was subsequently surgically removed, with no further evidence of recurrence on follow-up) [ 170 ]. In contrast, other extra-appendiceal gastrointestinal NETs, although rarer than appendiceal NETs in children, have a greater risk of recurrence [ 13 ].…”
