Rectovaginal endometriosis (RVE) is one of the most serious and incapacitating forms of presentation of this disease. Traditionally, medical treatment has not been considered effective for the majority of patients, being surgery the only reasonable and therapeutic choice in these cases. This exposes patients to a potentially serious morbidity, thus a careful evaluation should be done by a surgical board considering the impact of the disease as well as the quality of life of the patients. The main surgical techniques used are the shaving of the rectal wall affected by the endometriosic implants, the discoid excision of the front rectal wall, and the segmental intestinal resection, and there is no consensus concerning which is the most effective and suitable between them. The bibliography published in the last 10 years relating to the surgical treatment of RVE is being reviewed with the intention of updating the knowledge base about the topic and looking for common ground between different studies, allowing us to come closer to reaching a consensus about treatment for this pathology.

Esettanulmányukban a szerzők egy 27 éves beteg rectumot, szigmabelet és a spatium rectovaginalét érintő mélyen infi ltráló endometriosisának laparoszkópos műtéti kezeléséről számolnak be. A műtét során eltávolították az érintett vastagbélszakaszt, majd end-to-end anastomosist készítettek. Ezt követően a spatium rectovaginale és a hátulsó hü-velyboltozat mélyen infi ltráló endometriosiscsomóját CO 2 -lézer alkalmazásával excindálták. A kimetszett 12 cm hoszszú bélszakaszt transvaginalisan úgynevezett "natural orifi ce specimen extraction" technikával távolították el a hasüregből. Az első posztoperatív napon a rectumból utóvérzést észleltek, amit szintén laparoszkópia útján láttak el. Az irodalmi adatok áttekintése alapján megállapítják, hogy a "natural orifi ce specimen extraction" technika széles körben alkalmazható mélyen infi ltráló endometriosis miatt végzett műtétek során, és az általuk végzett laparoszkópos szegmentális bélreszekció vérzéses szövődménye laparoszkópia útján is biztonságosan ellátható. During surgery the affected bowel segment, the deeply infi ltrating nodule of the rectovaginal septum and the posterior vaginal wall were resected and the 12 cm long specimen was removed transvaginally. Postoperative bleeding was noted in the fi rst postoperative day, which was treated laparoscopically, aswell. This case history confi rms data from the literature showing that the natural orifi ce specimen extraction procedure can widely be applied during operations for deeply infi ltrating endometriosis and that laparoscopic anterior resection is a safe and feasible method for the treatment of colorectal deeply infi ltratnig endometriosis.
