Radial deformation by tissue Doppler imaging in multiple myocardial layers

Abstract: We have demonstrated that it is feasible to measure radial velocity, strain rate and strain in up to three individual layers in young healthy humans, and the diagnostic potentials should be tested on patient groups.

“…Clark and colleagues were the first to report the endocardial-to-epicardial Ecc gradient using tagging with 1.5-T MRI in 10 healthy hearts24. Similar findings were confirmed by STE, tissue Doppler imaging (TDI), phase-contrast magnetic resonance (PC-MR), and strain-encoded (SENC) CMR121325. Most studies carried out transmural analyses by dividing the myocardial wall into two layers (endocardial and epicardial)10262728.…”

“…Other than the well-known importance of global strain, layer-specific strain and distribution patterns of regional strain have not been investigated. Analyses of multiple-layer strain by echocardiography have been developed, and a transmural strain gradient demonstrated121314. However, layer-specific analyses of myocardial strain by FT-CMR have not been studied.…”

“…The commercially available EchopacPC software uses a weighted linear regression of the velocities over the SL (10). The true acquisition area with this software extends one-half SL above and below the ROI, as illustrated elsewhere (6,22). Care was thus taken not to place the ROIs within a distance of one-half SL from the epicardium and one-half SL from the endocardium.…”

“…Reference values for the normal LV rotation gradient from apex to base using echocardiography are thus needed. Increased interest has recently been shown for myocardial multilayer strain measurements (2,3,6,14,21,27). In contrast with methods averaging strain over the wall, multilayer analysis allows for more layer-specific information regarding the functional state of myocardial fibers at different depths of the wall.…”

“…In the present study, we aimed to correlate the potential differences in rotational velocities of individual LV segments with ventricular insertion sites or major heart vessels located in close proximity with the left ventricle. Considering recent interest in myocardial multilayer measurements, which provide more layer-specific information about the functional state of myocardium at different levels [5][6][7][8][9][10], separate measurements of rotational myocardial velocities for the inner (endocardial), middle (transmural) and outer (epicardial) layers of the LV wall were performed for 16 ventricular segments.…”
