Radial Artery as a Coronary Artery Bypass Conduit

Abstract: The 20-year patency rate of RA grafts is good, and not inferior to the ITA, especially when the conduit is used to graft a vessel with >90% stenosis. RA harvesting does not lead to hand or forearm symptoms, even at a very-long-term follow-up.

“…In framework of potential limb ischemic events, according to Gaudino, twenty-five patients underwent echo Doppler evaluation of the forearm circulation at a mean interval of 17.6±2.1 years from surgery and in the operated forearm, there was a significant increase in the diameter of the ulnar artery (2.44±0.43 mm vs. 2.01±0.47 mm; p<0.05). All other flow parameters were similar between the operated and control arms [9]. Although, this conduit is underused, despite numerous observational studies, which documented excellent RA patency of 89% at 10 years, and 86% at 20 years [10], which still constitute the first choice during the graft selection process.…”

“…In addition, there is an accumulating and strong body of literature showing better RA graft patency compared with SV graft patency, including Gaudino and colleagues' recent report [5] of 20-year angiographic data showing equivalent RA and left internal thoracic artery patency, which were both far superior to SV patency. Nonetheless, SV grafting still has a role in coronary artery bypass grafting (CABG).…”

“…From our point of view, there is not an ideal moment to do it. Some researchers think that PPM should be detected 6 to 12 months after surgery, because this variable determines long-term impact on survival [5]. However, taking into account that the patient…”

“…In other studies [24,25] patency rates were reported, but follow-up period was short. For the open technique more data are available in the literature [26,27]. Excellent patency rates have been reported after a long follow-up periods [26][27][28].…”

“…For the open technique more data are available in the literature [26,27]. Excellent patency rates have been reported after a long follow-up periods [26][27][28]. Since the long term results, so far, are at least equal (if not superior) and complication rates, as well as esthetic result is (at least in our study) very good after using open RA harvesting technique, we believe that it should be equally appreciated and not put aside as the less desirable technique.…”
