Quantitative analysis of the pulmonary wedge angiogram in congenital heart defects. Correlation with hemodynamic data and morphometric findings in lung biopsy tissue.

Abstract: SUMMARY At cardiac catheterization, 93 pulmonary wedge angiograms were performed in 85 patients with congenital heart defects. The pulmonary wedge angiogram was quantitatively analyzed and the findings were compared with hemodynamic features and morphometric assessment of lung biopsy tissue when available (27 patients). A pulmonary wedge catheter was directed into the right or left lower lobe to the origin of the posterobasal segment artery. The balloon was inflated and 0.3 mI/kg Renovist injected; then, the b… Show more

“…Intimal and medial hypertrophy, vascular remodelling, and neovasculogenesis lead to vascular occlusion and reduced pulmonary flow. Current PAH-specific therapies are targeted to re-establish pulmonary vasodilatation and reduce smooth muscle proliferation and hypertrophy in pulmonary arteries (6,7). Inflammation may be an important contributing factor to the progression of severe PAH.…”

“…These features can be visualised by wedge angiography and quantified by calculating tapering (rate of reduction in vessel diameter), background haze (contrast fluid intensity in peripheral lung fields) and circulation time 27. At present, reduction in fractal branching can be calculated using a fractal dimension algorithm on CT angiography 17.…”

“…Pulmonary angiography has served as the gold standard for assessing proximal pulmonary anatomy as well as for providing insight into the relative perfusion of the various lung segments [63]. It is an invasive procedure that requires placement of a right heart catheter (RHC) followed by the fluoroscopic visualization of contrast medium injected directly into the pulmonary arteries (Figure 6).…”
