Quantification of Phosphatidylethanols in Whole Blood as a Proxy for Chronic Alcohol Consumption, Using Ultra Performance Convergence Chromatography Tandem Mass Spectrometry

Abstract: The described method is suitable for the simultaneous quantitative analysis of the most abundant PEth homologues. Major advantages are low LLOQs, minimal sample volume and clean-up, and a short run time. The method is now available to monitor alcohol consumption in patients and has been incorporated in clinical practice and research.

“…The transitions for each PEth species (Table S1) are consistent with previous reports (Jones et al, 2011;Kummer et al, 2016;Nalesso et al, 2011;Van Der Nagel et al, 2018). Figure 2A depicts a total ion chromatogram for neat standards of all PEth homologues and Figure 2B shows extracted ion chromatograms (EICs) on a split axis for PEth homologues spiked together into a PEth-negative liquid whole blood sample, spotted onto a blood spot card, dried, and extracted.…”

“…The procedure has proven to be reliable and robust. The UPC2-MS/MS method has been reported as a reliable, flexible, and suitable method for PEth measurements (van der Nagel et al, 2018).…”

“…To minimalize the possibility of impurity, different batch numbers were used. The analysis is validated according to FDA/EMA validation guidelines (van der Nagel et al, 2018 ) and the assay is performed in the ISO15189 accredited Erasmus MC pharmacy laboratory. Based on PEth, three categories of drinking are identified; (1) light drinking/abstinence, (2) moderate alcohol consumption, and (3) excessive drinking.…”
