Objectives This prospective study analyses the 5‐year clinical outcome, patient satisfaction and the technical outcome of a 2‐implant overdenture on stud abutments. In addition, the influence of mandibular resorption and implant position on these outcomes was assessed. Material and methods Patients received two implants in a one‐stage delayed protocol. Final prosthesis on stud abutments was finalized after 3 months. Implant survival, bone loss, plaque and sulcus bleeding index, OHIP‐14, technical outcome and costs were assessed after 5 years. The Cawood–Howell classification defined the mandibular resorption. Inter‐implant distances and angles (frontal, sagittal) were digitally measured. The significance level was p < .05. Results Fifty‐six patients, 23 female and 33 males (mean age = 66.3, range 41–82), completed the follow‐up. No implants were lost. Radiographical crestal bone loss was on average 1.25 mm (standard deviation SD 1.06), bleeding index was 0.75 (SD 0.73), and plaque index was 1.15 (SD 0.75) and higher for low mandibles (p = .001). The OHIP‐14 reduction post‐connection and after 5 years was significant (p < .001). Five‐year maintenance required on average 6.7 (SD 4.8, range 0–25) interventions per patient. A larger inter‐implant distance was associated with fewer replacement of retention inserts (p = .034) and less interventions (p = .006). Larger frontal (p = .023) and sagittal (p = .046) inter‐implant angles (non‐parallelism) required more inserts. The maintenance cost negatively influences the patient satisfaction (p = .004). 83% of the stud abutments showed wear. Conclusion The implant overdenture on cylindrical stud abutments is a good and stable solution, irrespective of the resorption profile of the mandible. OHIP‐14 improved permanently up to 5 years but is negatively influenced by complication cost. A small inter‐implant distance, more frontal, sagittal inter‐implant divergence increased maintenance.

Objective: Oral health-related quality of life (QoL) is significantly influenced by the type of the prostheses that is used for prosthetic rehabilitation of patients with complete edentulism. This study aims to retrospectively evaluate the outcomes of mandibular implant-supported overdentures (MODs) through patients' perceptions by analyzing the results from OHIP-14 index. Methods: Relationships between OHIP-14 scores, and clinical and patient-based findings are investigated. Results: Forty-seven patients were included in the study. OHIP-14 scores ranged from 0 to 35, with a mean score of 5.81 (±6.89), indicating an overall "very high" quality of life. Patients with "very high" QoL had significantly lower pain scores than those with both "high" and "low" QoL (p=0.000, p=0.036 respectively). Further analysis revealed a positive correlation between total OHIP points and pain level (r= 0.738, p=0.000). Conclusion: Within the limitations of this study, it can be concluded that patients with complete edentulism of the mandible can effectively be rehabilitated with two implant-supported mandibular overdentures and be provided with a "very high" oral health-related QoL.

Purpose To investigate the influence of impaired masticatory function on the oral health‐related quality of life of conventional complete denture wearers before and after the transition to implant‐retained mandibular overdentures. Materials and Methods Masticatory function and oral health‐related quality of life were evaluated in 40 edentulous patients after 3 months of adaptation to complete dentures and after transition to implant‐retained mandibular overdentures. Masticatory function was evaluated through the swallowing threshold test with a standardized portion of an artificial test food (Optocal) to determine median particle size (×50) and homogenization index (B). After the test, the masticatory function outcomes were used to categorize patients in 2 groups according to chewing performance: (i) satisfactory or (ii) unsatisfactory. The cutoff value for ×50 was based on a preestablished value of 3.68 mm, whereas for the B index, median values were adopted for categorization. The oral health‐related quality of life was analyzed via the Oral Health Impact Profile for Edentulous Adults (OHIP‐EDENT) questionnaire. The Mann‐Whitney test was used to compare masticatory function outcomes and OHIP‐EDENT scores, and associations were checked using the Spearman correlation test and multiple linear regression. Results Masticatory function was significantly correlated with functional limitation (p = 0.02) and physical disability (p = 0.04) for complete denture wearers with unsatisfactory masticatory function. After the transition to implant‐retained mandibular overdentures, only the psychological discomfort domain (p = 0.02) was influenced by the masticatory function category. Multiple linear regression analysis revealed that only the functional limitation domain (p < 0.001) of the complete denture wearers was associated with masticatory function outcomes. Conclusions Implant‐retained mandibular overdentures eliminate the problems that lead to functional limitations, consequently improving masticatory function and oral health‐related quality of life of these individuals; however, patients with unsatisfactory masticatory function after transition to implant‐retained mandibular overdentures still experience more psychological discomfort.
