Total proctocolectomy with ileal pouch-anal anastomosis (IPAA) is the standard surgical treatment for patients with ulcerative colitis (UC). The purpose of this study was to investigate the long-term functional outcomes and quality of life (QOL) associated with hand-sewn and stapled IPAA. Ninety-one patients with UC had undergone IPAA using hand-sewn anastomosis with mucosectomy (32 patients) or stapled anastomosis (59 patients) from January 1988 to May 2010. Patients were evaluated according to patient characteristics, postoperative complications, functional outcomes and QOL. The QOL of patients were evaluated using the Medical Outcomes Study Short Form 36 (SF-36) and the Inflammatory Bowel Disease Questionnaire (IBDQ). Numbers of patients with colorectal cancer or dysplasia were significantly greater in the hand-sewn IPAA group (P , 0.01). These patients had longer disease durations and were older (both P , 0.01). There was no difference in the incidence of complications between the groups, except for a greater incidence of postoperative anal fistula in the stapled group (P ¼ 0.03). In the early postsurgery period, both the frequency of bowel movements and the rate of soiling were significantly higher in the hand-sewn group, but in a later period, there was no difference in these events .3 years after surgery. Total proctocolectomy with ileal pouch-anal anastomosis (IPAA), which was first described in 1978 by Parks and Nicholls, 4 has become established as the standard surgical treatment for patients with UC. The problems with IPAA are that the excision close to the anal sphincter may result in impaired anal function that may cause neurologic damage to the rectum, in turn resulting in urinary and sexual dysfunction. The anastomosis of ileal pouch leads to diarrhea and frequent bowel movements that may adversely affect patients' quality of life (QOL). Two different anastomotic techniques are generally used in IPAA: handsewn anastomosis with mucosectomy, which removes all rectal mucosa to the dentate line, and stapled anastomosis (Fig. 1). Lífsgæði eftir ristilbrottnám vegna sáraristilbólgu

4Brottnám á ristli og endaþarmi er mikilvaegur þáttur í með-ferð á sáraristilbólgu. Ábendingar fyrir bráðaaðgerð eru oftast bráð ristil bólga sem ekki raeðst við með lyfjum og sjaldnar rof á ristli eða blaeðingar. Ábendingar fyrir valaðgerðum eru skortur á svörun við lyfjameðferð, krabbamein eða krabbameinsáhaetta, þrenging á görn, fylgisjúkdómar utan garnarinnar og þegar börn þrífast ekki. 5,6 Líkur á þörf fyrir ristilbrottnám eru 31% laegri meðal þeirra sem hafa verið meðhöndlaðir með infliximab en með cyclosporíni. Hlutfall sjúklinga sem fer í ristilbrottnám er haerra í fyrsta "kasti" en síðari "köstum" (17,2% á móti 10,6%). Alvarleiki ristilbólgu og aldur eru mikilvaegir forspárþaettir fyrir ristilbrottnám. 7Hlutfall sjúklinga með sáraristilbólgu sem þarf á aðgerð að halda einu, 5 og 10 árum eftir greiningu var 4,9%, 11,6% og 15,6% árin 1955-70 og hefur farið laekkandi síðustu 6 áratugi. Meðal sjúk-linga sem greindust árin 2000-2010 var hlutfallið eftir eitt og 5 ár Tilgangur: Þónokkur hluti sjúklinga með sáraristilbólgu fer í ristilbrottnám. Markmið rannsóknarinnar var að kanna lífsgaeði þessara sjúklinga eftir aðgerð. Efniviður og aðferðir: Allir sjúklingar með sáraristilbólgu sem fóru í ristilbrottnám á Landspítala eða Sjúkrahúsi Akureyrar á árunum 1995-2009 og voru á lífi í upphafi rannsóknar voru í úrtakinu. 106 sjúklingar fengu senda þrjá spurningalista. SF-36v2 og EORCT QLQ-CR29 eru staðlaðir lífsgaeða listar þar sem spurt er um almennt viðhorf til heilsu og um einkenni frá endaþarmi eða stóma. Þriðji listinn innihélt starfraenar spurningar hannaðar af rannsóknaraðilum. Niðurstöður: Svör bárust frá 83 (78%), 45 körlum (54%) og 38 konum (46%). Meðalaldur við aðgerð var 45 ár (10-91 ár). Fjörutíu og fjórir (53%) höfðu garnarauf, 28 (34%) innri garnapoka (IPAA) og 11 (13%) tengingu mjógirnis í endaþarm. Hjá sjúklingum þar sem endaþarmur var fjarlaegður lýstu 37% breytingum á þvaglátum og 46% á kynlífi eftir aðgerð. 75% svarenda með innri garnapoka lýstu haegðaleka en hann var vaegur samkvaemt Wexner-skala hjá 83% þeirra. Enginn munur var á lífsgaeðum þátttakenda og almenns þýðis samkvaemt SF-36v2. Sjúklingar höfðu litlar áhyggjur af heilsu, líkamsímynd eða þyngd og höfðu aðeins mild einkenni samkvaemt EORTC QLQ-CR29. Ályktanir: Algengt var að breytingar yrðu á þvaglátum og kynlífi eftir aðgerð þegar endaþarmur var fjarlaegður. Haegðaleki hjá þeim sem fengu innri garnapoka virtist mun algengari en búist var við. Ekki var marktaekur munur á lífsgaeðum þeirra sem höfðu farið í aðgerð og almenns þýðis. Niðurstöður rannsóknarinnar eru mikilvaegar þegar verið er að upplýsa sjúklinga um aðgerðarmöguleika þar sem brottnám á ristli eða það að hafa stóma virðist ekki skerða lífsgaeði. Á G R I P2,3% og 7,6%.8 Aðalaðgerðin vegna sáraristilbólgu er brottnám á ristli og endaþarmi með endagarnarauf. Stómapokinn getur verið óþaegilegur og hindrað þá sem þurfa að hafa hann. Breytingar á meðferðinni hafa því miðast við það að losa sjúklinga við stóma-pokann. Ef endaþarmurinn er ekki sjúkur er haegt að skilja hann eftir og...

Bevezetés: A colitis ulcerosa sebészi kezelésében egyre szélesebb körben alkalmazzák a laparoszkópos technikát, azonban hosszú távú magyarországi eredményekkel eddig még nem rendelkeztünk. Célkitűzés: A szerzők célja a hagyományos és a minimálisan invazív módszerrel operált betegek műtéti és 47,8 hónapos utánkövetési eredményeinek összehasonlítása. Módszer: 2005. január 1. és 2014. december 31. között összesen 56 beteg került műtétre colitis ulcerosa diagnózissal, akik közül 20-at sürgősséggel, 36-ot tervezetten műtöttek. Laparoszkópos technikával 33, nyitottan 23 műtétet végeztek. Eredmények: A perioperatív időszakban az ápolási idő, passzázsmegindulás, az intenzív osztályos és transzfúziós igény és szövődmények terén a csoportok között különbséget nem észleltek. Hosszú távú szövődmények tekintetében az intestinalis obstrukció, a szeptikus állapot és az egyéb komplikációk előfordulása szignifi kánsan alacsonyabb volt a laparoszkópos műtéten átesett betegek csoportjában. Mindkét csoport életminősé-gében szignifi káns javulást tapasztaltak a műtétet követően. Következtetések: A colitis ulcerosa kezelésében a laparoszkópos módszer biztonságosan alkalmazható, amely jó életminőséget, kedvezőbb kozmetikai eredményt biztosít és hosszú távon alacsonyabb a szövődmények aránya a nyitott műtétekkel összehasonlítva. Aim: The aim of the authors was to compare the mean 47.8-month follow-up results of patients treated with conventional and minimally invasive surgical methods. Method: Between January 1, 2005 and December 31, 2014, 56 patients were treated with surgery (20 emergencies, and 36 elective cases). Laparoscopy was used in 33 and conventional method in 23 cases. Results: There was no difference between the two groups in hospital and intensive care unit stay, bowel function recovery, need for transfusion, and complications during the perioperative period. Regarding long-term complications, the occurrence of intestinal obstruction, septic condition and other complications were signifi cantly fewer in the laparoscopy group. The quality of life improved in both groups after the surgery. Conclusions: Laparoscopy can be used safely; it provides good quality of life and better cosmetic results, and the long-term rate of complications is lower as compared to open surgery.
