Quality of care of patients hospitalized with acute coronary syndromes

Abstract: There were delays in performing ECG and administering thrombolysis to patients who presented to emergency departments with ACS. Improvement is warranted in use of non-invasive procedures for identifying high-risk patients who may benefit from coronary revascularization as well as use of serum lipid measurements, ACE inhibitors and cardiac rehabilitation.

“…7 Other recommendations that were adhered to less than 80% of the time included discharge prescription of angiotensinconverting enzyme inhibitors (ACEI), measurement of serum lipids, timely ECG and coronary angiography. The authors suggested that quality improvement might follow the implementation of a guideline-linked audit and feedback process, critical care paths, targeted professional tuition or a combined educational approach.…”

“…Despite study limitations similar to those discussed in our companion report on quality of care of acute coronary syndromes, 16 the present investigation reveals opportunities for improving quality of in‐hospital care of patients admitted with CHF to three teaching hospitals in Brisbane, Australia. Importantly, there is underutilization of key pharmacological interventions similar to those reported in Australian 11,12 and international studies 8−10 (Table 5).…”

“…At the time of the present study, the hospitals served a metropolitan catchment population of 1.2 million residents. Hospital characteristics have been described elsewhere 16 . Baseline data were collected prior to implementation of systematic quality‐improvement interventions as part of the Brisbane Cardiac Consortium Clinical Support Systems Program (see for full details of the Brisbane Cardiac Consortium Leadership Group) 17…”

“…Completion of an electrocardiogram (ECG) is a rapid and non-invasive method of evaluating possible cardiac ischemia in chest pain patients. Previous research has determined that timely performance of an ECG is an indication of quality management of patients that present with acute coronary syndromes, and guidelines recommend that an ECG be performed and shown to an experienced physician within 10 min for patients with suspected acute coronary syndrome (3,4). Its effectiveness as a diagnostic tool and its ease of completion make the ECG It has been recognized that appropriate care is not evenly distributed to all patients.…”
