Background: Regulatory bodies recommend inconsistent ejaculatory abstinence lengths before semen analysis. The literature exploring the effect of ejaculatory abstinence length on the outcomes of intracytoplasmic sperm injection is scarce. Objective: To study the influence of ejaculatory abstinence length on semen quality and intracytoplasmic sperm injection outcomes. Materials and methods: This prospective cohort study included 818 patients undergoing conventional semen analysis from October 2015 to October 2016, in a private university-affiliated IVF centre. Generalized linear models adjusted for potential confounders were used to investigate the associations between ejaculatory abstinence length and seminal parameters and intracytoplasmic sperm injection outcomes. Results: Increasing ejaculatory abstinence length was positively correlated with semen volume, sperm concentration, total sperm count, total motile sperm count and sperm DNA fragmentation index. Significant inverse correlations were observed between ejaculatory abstinence length and fertilization rate, blastocyst formation rate, implantation rate and pregnancy rate. A discriminant analysis showed a mean ejaculatory abstinence length in the positive pregnancy group of 3.14 AE 1.64 days and 4.83 AE 3.66 days in the negative pregnancy group. A cut-off point was established halfway between ejaculatory abstinence length averages, at 4 days. The ejaculatory abstinence ≤4 days group showed significant lower semen volume, sperm concentration, total sperm count and total motile sperm count compared to ejaculatory abstinence > 4 days group. The ejaculatory abstinence ≤ 4 days group showed significant lower sperm DNA fragmentation index, and higher rates of fertilization, high-quality embryos on day 3, blastocyst development, implantation and pregnancy compared to ejaculatory abstinence > 4 days group. The implantation rate was significantly higher and the pregnancy rate tended to be higher with one day of ejaculatory abstinence, compared to 2-4 days of ejaculatory abstinence. Conclusions: Ejaculatory abstinence periods of >4 days have a detrimental effect on sperm DNA and intracytoplasmic sperm injection outcomes. One day of ejaculatory abstinence significantly improves implantation rate and tends to increase pregnancy rate, compared to 2, 3 and 4 days of ejaculatory abstinence.

A detailed sperm motility study for 24 h after collection was done. The level of calcium and magnesium in seminal plasma during this period was also seen to understand the role of these electrolytes on sperm motility. Good care was taken in selection of subjects (young and healthy), collection and pre-physical analysis of sample. Subjects (healthy, 18-25 years) were to maintain abstinence (2-5 days). Collection of sample into wide mouthed container provided was done next to laboratory by masturbation at 8 am ± 30 m. The total study was done in one month period. Samples were studied at ½, 1, 2, 4, 8, 12 and 24 h time for percentage of (1) total sperm motility, (2) active and progressive and (3) non-progressive motility and seminal plasma calcium and magnesium. Total sperm motility deteriorated to 50% in 8 h whereas active and progressive type reduced to 50% in 4 h. At 24 h total sperm motility was 9.8% and active and progressive motility 3.0%. Level of calcium (27.2 mg/dl) and magnesium (13.54 mg/dl) in seminal plasma did not show any significant changes during study period from that of at ½ h. The study concluded that electrolytes under study were not responsible for the decrease in motility during study period.

ObjectivesMale infertility is on the rise. Artificial insemination is an option in many cases like oligozoospermia or oligoasthenozoospermia. Homologous insemination is helpful for some couples in whom sperm count is low. This study was aimed to understand the most suitable portion of split ejaculate for insemination and also the level of calcium and magnesium in each split.Materials and methodsA total number of 31 normal and clinically healthy adults participated in this study. They were instructed to maintain abstinence for 2-5 days prior to sample collection in three splits. Each split was evaluated as if it were a whole sample, following WHO criteria. Seminal plasma was separated. Calcium and magnesium levels were measured in seminal plasma and spermatozoa of each split.ResultsThe split ejaculate study revealed that the first portion contained a higher number of spermatozoa with better motility than the 2nd and 3rd splits. Similarly, the level of calcium and magnesium in seminal plasma and spermatozoa was greater in the first split, followed by 2nd and 3rd splits.ConclusionThe presented split ejaculate study has shown the first ejaculate as superior in quality in terms of total sperm count and percentage of active motility. The first split of ejaculation is recommended for homologous artificial insemination in case of oligozoospermia and oligoasthenozoospermia. This study has also shown that a major portion of two important elements, calcium and magnesium, is contributed by the prostate gland.
