Quality Measure Performance in Small Practices Before and After Electronic Health Record Adoption

Abstract: Introduction:To date, little research has been published on the impact that the transition from paper-based record keeping to the use of electronic health records (EHR) has on performance on clinical quality measures. This study examines whether small, independent medical practices improved in their performance on nine clinical quality measures soon after adopting EHRs.Methods:Data abstracted by manual review of paper and electronic charts for 6,007 patients across 35 small, primary care practices were used to… Show more

“…However, electronic clinical data alone-without specific quality metrics and guidelines for use-may fall short of recognition and management of malnourished patients. 15 These constraints limit the ability to meet evidence-based care standards and leverage the EHR in addressing gaps in care. 16 Therefore, a more integrated approach is needed to assist all clinicians on the interprofessional care team in capturing and leveraging clinical data for improved decision making and for delivering cost effective quality care.…”

“…29 This is because many performance-incentive programs have specific requirements, but lack guidance on parallel improvements in clinical processes as has been demonstrated across various settings of care. 15,30 measures, meet evidence-based care standards, and leverage EHR technology for addressing gaps in care. 16 Therefore, new approaches are needed to assist clinicians in leveraging clinical data for improved decision making in delivering quality care.…”

“…Renaissance “ monality ” (Wienpahl, 1971 )—modality with the major/minor triadic principle of organization—presented the first style of tonal rather than modal integration of harmony. It flourished in the genre of frottola, popular since 1490s (Prizer, 1975 ).…”

“…Electronic health record (EHR), used during clinical encounters, has become increasingly popular all around the world [8,9]. Information and communication technologies defined as "electronic health" may also increase the capability to identify target groups of patients for specific interventions, improving clinical healthcare quality, especially for specific patient groups [10,11]. Clinical information, called "personal health record", refers to the collection of data about an individual's health and health care, stored electronically [9][10][11].…”
