Quality improvement programme to achieve acceptable colonoscopy completion rates: prospective before and after study

Abstract: Problem A large audit of colonoscopy in the United Kingdom showed that the unadjusted completion rate was 57% when stringent criteria for identifying the caecum were applied. The caecum should be reached 90% of the time. Little information is available on what units or operators need to do to improve to acceptable levels. Design Quality improvement programme using two completed cycles of audit. Setting Endoscopy department in a university linked general hospital in northeast England. Key measures for improveme… Show more

“…5 Furthermore, it hinders the cecal insertion rate (greater than 90%), which is an essential component of colonoscopy quality assurance. 12 Among the available bowel preparation formulas, the combination of 2-L PEG+Asc was preferred compared to other formulas because it required a lower ingestion volume than 4-L PEG, while providing similar bowel cleansing potency 7 and better compliance due to improved flavor.…”

“…Rates are lower in some settings 6,20,21, but there is evidence that rates can be improved by quality improvement programs 21 . We recommend that all endoscopists should aim to meet this target, regardless of specialty, training, or experience.…”

“…24 Others have also succeeded using local resources discussing performance results and intervention, like Imperiali et al 25 in Northern Italy, where CIR was improved from 85% to 90%. Ball et al 26 in England achieved an impressive improvement on CIR from 60% to 94% by a combination of organisational measures which included more time per colonoscopy, letting the most successful endoscopists perform more colonoscopies, retraining the less successful and even advising some to give up colonoscopy entirely.…”
