Clinical educators’ skills and qualities in allied health: a systematic review

Background The skills and qualities of effective clinical educators are linked to improved student learning and ultimately patient care; however, within allied health these have not yet been systematically summarised in the literature. Aims To identify and synthesise the skills and qualities of clinical educators in allied health and their effect on student learning and patient care. Method A systematic search of the literature was conducted across five electronic databases in November 2017. Study identification, data extraction and quality appraisal were performed in duplicate. Qualitative and quantitative data were extracted separately but analysed together using a thematic analysis approach whereby items used in quantitative surveys and themes from qualitative approaches were interpreted together. Results Data revealed seven educator skills and qualities: (i) intrinsic and personal attributes of clinical educators; (ii) provision of skillful feedback; (iii) teaching skills; (iv) fostering collaborative learning; (v) understanding expectations; (vi) organisation and planning; and (vii) clinical educators in their professional role. Across all themes was the concept of taking time to perform the clinical educator role. No studies used objective measurements as to how these skills and qualities affect learning or patient care. Discussion and conclusion Despite much primary evidence of clinical educator skills and qualities, this review presents the first synthesis of this evidence in allied health. There is a need to examine clinical education from new perspectives to develop deeper understanding of how clinical educator qualities and skills influence student learning and patient care.

ObjectivesTo synthesise the findings of qualitative research exploring parents’ experiences, views and decisions about becoming pregnant following a perinatal death or fetal loss.DesignSystematic review and meta-synthesis of qualitative research.Data sourcesMedline, Web of Science, CINAHL, PsycINFO, ASSIA, Embase, PUBMED, Scopus and Google Scholar.Eligibility criteriaNine electronic databases were searched using predefined search terms. Articles published in English, in peer-reviewed journals, using qualitative methods to explore the experiences and attitudes of bereaved parents following perinatal or fetal loss, were included.Data extraction and synthesisQualitative data relating to first-order and second-order constructs were extracted and synthesised across studies using a thematic analysis.Results15 studies were included. Four descriptive themes and 10 subthemes were identified. The descriptive themes were: deciding about subsequent pregnancy, diversity of reactions to the event, social network influences, and planning or timing of subsequent pregnancy. The decision to become pregnant after death is complex and varies between individuals and sometimes within couples. Decisions are often made quickly, in the immediate aftermath of a pregnancy loss, but may evolve over time. Bereaved parents may feel isolated from social networks.ConclusionsThere is an opportunity to support parents to prepare for a pregnancy after a fetal or perinatal loss, and conversations may be welcomed at an early stage. Health professionals may play an important role providing support lacking from usual social networks.PROSPERO registration numberCRD42018112839

It has been our practice to publish a State of the Science themed issue in the August issue of JHL. Over the years authors have expressed confusion about what a State of the Science research review is and how to write one. The purpose of this article is to address those questions, focusing on the nature, purpose, and usefulness of dedicating one of our four annual issues to this topic. We agree with Horsley's (2019) assessment: "The findings of reviews are far too important for advancing a field to be conducted with any less integrity than primary research" (p. 56).We have defined the State of the Science as a review of the most comprehensive and up-to-date published evidence available, at a given time. "State of the Science articles refers to the general quality and rigor of the review" (Carter-Templeton, 2022, p. 8). It is not a strategy or particular method for reviewing the scholarly literature; rather, it refers to the depth and the rigor of the process undertaken by those reviewing the literature. As the name implies, it refers to both the current relevancy and the high level scholarly review activity. Conducting this type of research is time-consuming, requiring a knowledgeable team of researchers using established guidelines. The goal of this type of article is to provide a picture of what is known about a topic at a specific time, including a critique of the methods used to create that evidence and an identification of gaps in the evidence. This type of research review provides evidence that can support or negate the assumptions underlying our current understandings and practices, in this way creating new knowledge. Additionally, State of the Science papers provide clinicians, educators, researchers, and policy makers with reliable and relevant evidence with which they can make clinical decisions, develop future research, and develop programs and policies. They are incredibly important to the development of knowledge and the development evidence-based practices.
