Examinations for early detection of diseases (called U-Untersuchungen in Germany) are among the most important prevention measures at childhood age. According to KiGGS Wave 2 data, participation rates are over 95% for most of these examinations. 99.7% and 99.6% of children, respectively, who had reached the recommended age for these examinations participated in the U1 and U2 examinations, 98.0% and 98.1%, respectively, the U8 and U9 examinations. Participation rates for children from families with low socioeconomic status and those with a two-sided migration background are slightly lower. A comparison with previous KiGGS waves shows that the utilization of early detection examinations has increased significantly over the last ten years. During this time, social differences which were previously pronounced have decreased.

“…Dementsprechend traten die schichtspezifi schen Unterschiede für die Teilnahmen beim Berliner Einschulungsjahrgang 2007 für komplette U1-U8 (niedriger Sozialstatus: 64 % vs. mittlerer: 80,0 % vs. hoher: 84,0 %) [ 30 ] und im Berliner Bezirk Pankow [ 31 ] für durchgeführte U9 für die Jahrgänge 2013 (niedriger Sozialstatus: 83,6 % vs. mittlerer: 86,3 % vs. hoher: 87,6 %), 2014 (niedriger Sozialstatus: 73,7 % vs. mittlerer:84,6 % vs. hoher: 86,9 %) und 2015 (niedriger Sozialstatus: 76,9 % vs. mittlerer:87,1 % vs. hoher: 89,5 %) aufgrund der heterogeneren Sozialstruktur dieser großstädtischen Untersuchungsgebiete wesentlich deutlicher hervor als im Stadtgebiet XXX. Zusammenfassung Ziel der Studie U-Vorsorgeuntersuchungen und Impfungen sind zentrale Vorsorgemaßnahmen des Kindesalters, wobei bisher keine quantitativen Angaben zu deren wechselseitiger Akzeptanz vorlagen. Ziel dieser Studie war die Quantifizierung dieser Verteilungsmuster und Identifikation von Assoziationen zwischen Vorsorgeuntersuchungen und definierten Routineimpfungen bei Einschulungskindern unter Berücksichtigung des sozialen Umfeldes. Methodik Im Rahmen der Schuleingangsuntersuchungen 2015 wurden unter anderem die dokumentierte U-Vorsorgeuntersuchungen und Routineimpfungen auf Stadtebene erfasst. Begleitend wurde ein freiwillig auszufüllender Proxy-Fragebogen an die Fürsorgeberechtigten der Einschulungskinder ausgegeben, um ergänzend Merkmale der familiären Lebensverhältnisse zu erfassen. Unter Verwendung von SPSS 25.0 wurden die Teilnahmequoten aller U-Vorsorgeuntersuchungen und definierter Routineimpfungen nebst spezifischer Kreuzprävalenzen und Risikoschätzer errechnet. Ergebnisse Von den 407 Einschulungskindern wurden bei 76,8% der Vorschulkinder beide Präventionsmaßnahmen vollständig durchgeführt, wobei innerhalb der gesamten Studienkohorte von 280 (68,8%) ein Zusatzfragebogen abgegeben wurde. Am häufigsten hatten in Deutschland geborene Kinder zu 70,3% und zu 69,7% mit deutscher Nationalität komplette Vorsorgeuntersuchungen mit zugleich vollständigem Impfstatus. Hinsichtlich des familiären Umfeldes hatten Kinder mit Geschwistern (17,5%), Kinder mit getrenntlebenden Elternteilen (13,3%) und Familien mit bis zu 4 näheren Angehörigen (7,1%) deutlich seltener alle Vorbeugemaßnahmen komplett als andere. Die stärksten Assoziationen in Hinblick auf unvollständige Vorsorgeuntersuchungen waren für ein nichtdeutsches Geburtsland des Kindes (OR=7,95) und für eine nichtdeutsche Nationalität mindestens eines Elternteiles (OR=4,52) vorzufinden. Schlussfolgerungen Die identifizierten Verteilungsmuster und Assoziationen lieferten neue Erkenntnisse für die Etablierung von risikogruppenspezifischen Maßnahmen zur Steigerung der Akzeptanz sowohl bei U-Vorsorgeuntersuchungen als auch in Hinblick auf definierte Routineimpfungen. Dadurch kann eine bedarfsoptimierte Gesundheitsversorgung bei Kindern im Sinne eines sozialkompensatorischen Ansatzes ermöglicht werden.

COVID-19 Pandemic and Families´ Utilization of Well Child Clinics and Pediatric Practice Attendance in Germany

ObjectiveThe COVID-19 pandemic and the measures implemented to stop the pandemic had a broad impact on our daily lives. Besides work and social life, health care is affected on many levels. In particular, there is concern that attendance in health care programs will drop or hospital admissions will be delayed due to COVID-19-related anxieties, especially in children. Therefore, we compared the number of weekly visits to 78 German pediatric institutions between 2019 and 2020. ResultsWe found no significant differences during the first ten weeks of the year. However, and importantly, from April, the weekly number of visits was more than 35% lower in 2020 than in 2019 (p=0.005). In conclusion, the COVID-19 pandemic seems to relate to families´ utilization of well-child clinics and pediatric practice attendance in Germany. Of note is that an Austrian study shows that delays in accessing acute pediatric health care may outweigh the risks of COVID-19 because diagnostics and treatments of severe diseases might be delayed during the pandemic.
