Quadritherapy vs standard tritherapy immunosuppressant regimen after heart transplantation: A propensity score–matched cohort analysis

Abstract: After heart transplant, adding everolimus (EVL) to standard immunosuppressive regimen mostly relies on converting calcineurin inhibitors (CNIs) into EVL. The aim of this study was to describe the effects of combining low-dose EVL and CNIs in maintenance immunosuppression regimen (quadritherapy) and compare it with standard tritherapy associating standard-dose CNIs, mycophenolate mofetil, and corticosteroids. In the 3-year registry cohort of heart transplanted patients, those who received quadritherapy were com… Show more

“…The effect of HCQ/AZI on clinical outcomes was assessed in the weighted inflated population. This method aims to mitigate non-randomized allocation of treatment and was used previously in other retrospective studies including one in COVID-19 setting [14][15][16]. Kaplan-Meier curves were used to compare HCQ/AZI group and control group on time to the first outcome with a censorship at 28 days.…”

“…We could not assess the efficacy of desensitization on DSA after HTx, specifically de novo DSA, due to the lack of sera in the historical control cohort. Maintenance immunosuppression regimen has been subject to change in the past 15 years with tacrolimus suggested instead of ciclosporin ( 19 ) and mammalian target of rapamycin (mTOR) inhibitors in addition to, or in place of, calcineurin inhibitors ( 12 , 20 ). These may impact the treatment effect observed due to desensitization.…”

“…Except for postoperative desensitization, the standard immunosuppressive regimen did not change during the study period (2004)(2005)(2006)(2007)(2008)(2009)(2010). As previously described (12), in the postoperative period it included thymoglobulin induction therapy from day 0 to day 4 (rabbit ATG, Thymoglobuline, Genzyme, Lyon, France), methylprednisolone bolus infusion on day 1, ciclosporine after day 1, and mycophenolate mofetil (MMF) after day 4. Corticosteroids were converted to oral form starting on day 4, with a dosage of 1 mg/kg initially.…”
