Pyometra due to Pseudomonas aeruginosa

Abstract: Pyometra is an unusual presentation of uterine infection, this condition is characterized by purulent fluid within the uterine cavity. In this report we describe pyometra due to Pseudomonas aeruginosa in a 90-year-old woman who presented with abdominal pain, nausea with emesis and diarrhea. She was evaluated at an outside emergency department and computed tomography (CT) revealed a large, complex, cystic mass measuring 9.2 cm (cm) in greatest diameter. Given her age, the diagnosis of mal… Show more

“…Since pyometra more commonly affects older patients with impaired activities of daily living, it is often mistaken for a urinary tract infection and can be easily overlooked. Furthermore, half of these cases have been reported to be asymptomatic [ 8 ]. In the present case, since the source of fever could not be identified on initial examination and there was no pyuria, a CT scan was performed on the same day, which allowed for an early diagnosis.…”

“…The most common causative organisms are aerobic bacteria, such as Streptococcus spp., Escherichia coli, and Enterococcus spp., and anaerobic bacteria, such as Bacteroides spp [ 9 ]. Pseudomonas aeruginosa and Mycobacterium tuberculosis have also been reported, indicating diversity in the causative organisms for pyometra [ 8 ]. To the best of our knowledge, this is the first case of pyometra by Achromobacter xylosoxidans was first isolated in Japan in 1971 from an ear discharge sample from a patient with chronic otitis media [ 10 ].…”

“…Pyometra describes the accumulation of purulent fluid in the uterus, and it is relatively rare; hence, we clinicians do not often consider it in the differential diagnosis of a febrile patient. The most common cause of pyometra comprises stenosis of the cervix caused by events such as malignancy, radiation-induced cervicitis and atrophic cervicitis ( 1 ). Thus, diverticulitis is one of the rarest causes, and there are only a few reported cases so far.…”

“…8 However, there are also several case reports regarding the etiology of other bacteria and it is also known that in 8-17% of cases sterile culture results are encountered. 9,10 Pyometra occurs due to obstruction in the drainage channels of the uterine cavity. Cervical stenosis most often involves the internal os.…”

“…The resulting pus will accumulate in the uterus and form pyometra. 9,10 It is known that the length of use of IUD is related with the risk of infection. An infection that occurs due to IUD causes the occurence of endometritis.…”
