Pyoderma gangrenosum associated with active chronic hepatitis: report of two cases

Abstract: In two patients, active chronic hepatitis was complicated by the development of pyoderma gangrenosum. The favorable response of the pyodermatous lesions to azathioprine therapy suggests that this drug may be of value in treating this disorder in patients for whom corticosteroid therapy produces no benefit or is contraindicated because of side effects. Azathioprine, too, is a potentially toxic drug and may need to be discontinued. Possible causal relationships between pyoderma gangrenosum and active chronic hep… Show more

“…To our knowledge, our study is the first large case series that has found viral hepatitis as a significant comorbidity for PG. There have been a few single case reports on PG and viral hepatitis, 19–21 leading to the hypothesis that chronic liver disease, by compromising immunological functions, predisposes to other immunological conditions including PG. Seven of the patients (6·8%) had underlying HCV and two (1·9%) had an autoimmune hepatitis.…”

“…6 In 1976, two cases of pyoderma gangrenosum associated with chronic active hepatitis were reported, although the aetiology of hepatitis was not known. 7 These patients may have had Wilson disease. Pyoderma gangrenosum can also be associated with other underlying conditions like seropositive rheumatoid arthritis, myeloproliferative disorders and vasculitis, 8 which were not the possible causes in our case.…”
