This article addresses the relationship between Public-Private Partnerships (PPP) and the sustainability of public spending in smart hospitals. Smart (technological) hospitals represent long-termed investments where public and private players interact with banking institutions and eventually patients, to satisfy a core welfare need. Characteristics of smart hospitals are critically examined, together with private actors’ involvement and flexible forms of remuneration. Technology-driven smart hospitals are so complicated that they may require sophisticated PPP. Public players lack innovative skills, whereas private actors seek additional compensation for their non-routine efforts and higher risk. PPP represents a feasible framework, especially if linked to Project Financing (PF) investment patterns. Whereas the social impact of healthcare investments seems evident, their financial coverage raises growing concern in a capital rationing context where shrinking public resources must cope with the growing needs of chronic elder patients. Results-Based Financing (RBF) is a pay-by-result methodology that softens traditional PPP criticalities as availability payment sustainability or risk transfer compensation. Waste of public money can consequently be reduced, and private bankability improved. In this study, we examine why and how advanced Information Technology (IT) solutions implemented in “Smart Hospitals” should produce a positive social impact by increasing at the same time health sustainability and quality of care. Patient-centered smart hospitals realized through PPP schemes, reshape traditional healthcare supply chains with savings and efficiency gains that improve timeliness and execution of care.

This paper describes the modeling efforts devoted by the Ministry of Health and Social Services of Québec, Canada (MSSS), to ensure the long-term bearability of their care system. To this end, it studies the relationships between four entities that self-regulate and interact to form the complex care-providing system: (1) universities; (2) hospitals and doctors; (3) the ministry; and (4) society. The first phase of this research focuses on modeling such relationships and relies on the system dynamics methodology to adequately capture the long-term dynamics of the system. The methodology encompasses three phases: (a) determination of the critical variables and parameters of each entity; (b) development of the causal diagram of each entity; and (c) integration of the individual causal diagrams to form the global system diagram. The final casual model illustrates and explains the relationships between all the entities and constitutes an excellent tool to support experts during discussions or focus groups where critical variables that positively or negatively affect the system can be evaluated. We intend to enrich this casual model in a further phase of the project, which will hopefully lead to a simulation and scenario analysis tool that can be used to support managers in their long-term decision-making process.

Issues such as maintaining the security and integrity of data in digital healthcare are growing day-by-day in terms of size and cost. The healthcare industry needs to work on effective mechanisms to manage these concerns and prevent any debilitating crisis that might affect patients as well as the overall health management. To tackle such critical issues in a simple, feasible, and symmetrical manner, the authors considered the ideology of cybernetics. Working towards this intent, this paper proposes a symmetrical model that illustrates a compact version of the adopted ideology as a pathway for future researchers. Furthermore, the proposed ideology of cybernetics specifically focuses on how to plan the entire design concept more effectively. It is important for the designer to prepare for the future and manage the design structure from a product perspective. Therefore, the proposed ideology provides a symmetric mechanism that includes a variety of estimation and evaluation techniques as well as their management. The proposed model generates a symmetric, variety-issue, reduced infrastructure that can produce highly effective results due to an efficient usability, operatability, and symmetric operation execution which are the benefits of the proposed model. Furthermore, the study also performed a performance simulation assessment by adopting a multi-criteria decision-making approach that helped the authors compare the various existing and proposed models based on their levels of effectiveness.
