Pupil evaluation in addition to Glasgow Coma Scale components in prediction of traumatic brain injury and mortality (Br J Surg 2012; 99(Suppl 1): 122–130)

Abstract: Simple and selective: An efficient organocatalytic enantioselective method for the synthesis of substituted‐3‐hydroxyoxindole derivatives with a quaternary chiral carbon has been developed. The cinchona alkaloid derived catalyst catalyses the Friedel–Crafts‐type addition of indole derivatives to the isatin derivatives under mild conditions to provide substituted 3‐hydroxyoxindoles in good to excellent yields with high enantioselectivity (see scheme).

“…Pupillometry has become standard of care in many ICUs due to its accuracy in evaluation and predicting outcome [13]. Using pupillary reactivity in combination with individual motor response has proved to outweigh the full GCS in predicting outcome [14]. In the subacute phase, a structured neurological examination can determine prognosis and identify patients who can benefit from neurorehabilitation.…”

“…Data related to patient baseline demographic and clinical characteristics extracted from medical records, including age, sex, smoking history, alcohol consumption history, past medical history, cause of injury, pupil parameters, and body mass index (BMI), etc. Pupil evaluation was referred to the previous literature (Supplementary Table 1) (6,12,13). Pupil reactivity was coded into three subgroups: (1) brisk (both pupils constricted identically with light shone into either eye alone); ( 2) sluggish (delayed pupil constriction in reaction to light); ( 3) fixed (no response to a light stimulus, regardless of which eye was being stimulated).…”

“…Our study analyzed potential pyroptosis-related genes in TBI, which laid the foundation for subsequent clinical study. The Glasgow Coma Scale (GCS), pupil parameters and Helsinki CT score are often used to describe brain injury and evaluate its prognosis (6,7). The pathophysiological process secondary to trauma is important and complex, and many molecules are involved to influence the progression and outcome of the disease.…”
