Punctate Porokeratotic Keratoderma

Abstract: Extensive review of published reports of punctate keratoderma has revealed only one similar case to that reported in the present paper. This represents a diffuse involvement of the palms and soles with pinpoint keratotic plugs and pits which were proven to be parakeratotic columns histologically identical with the cornoid lamellae of porokeratotic conditions. The clinical and histologic features which set this form of keratoderma apart from other forms of punctate keratoderma are discussed.

“…Porokeratosis punctata palmaris et plantaris is clinically and histopathologically similar to the lesions on the palms and soles in case 2. The gross appearance of the lesions was reminiscent of the ‘keratotic projections’ or ‘keratotic plugs’ described in porokeratosis punctata [16,17,18,19]. Histopathologically, some keratinocytes had perinuclear haloes, which could simulate the vacuolisation that is typical of porokeratosis.…”

“…Although spiny keratoderma is considered by some to be a cutaneous marker of malignancy in genetically predisposed persons [6], in no case has its relationship with malignant neoplasms been established [2,[5][6][7][8]. The few cases reported, together with the fact that in our case the neoplasm was detected years after the onset of dermatosis, and the persistence of the lesions despite oncotherapy all suggest a lack of association.…”

“…The confusing terms used to describe it have led to difficulty in its classification. Brown [1] first described a similar case in 1971, calling it 'punctate kerato- derma' and 2 years later Herman [2] reported a case of 'punctate porokeratotic keratoderma' in a 50-year-old man. Other terms include 'punctate porokeratosis' [3], 'punctate porokeratosis of the palms and soles' [4] and 'hyperkératose palmoplantaire filiforme' [5,6].…”

“…1 Punctate porokeratotic keratoderma (palmaris et plantaris), spiny keratoderma and punctate porokeratosis (palmaris et plantaris) are frequently referred to indiscriminately as if they referred to the same assumption. 2-4 More recent publications have however highlighted major distinctions between these descriptions. 5 …”
