Pulsed Acoustic Cellular Expression (PACE) Reduces Capsule Formation Around Silicone Implants

Abstract: This journal requires that authors assign a level of evidence to each submission to which Evidence-Based Medicine rankings are applicable. This excludes Review Articles, Book Reviews, and manuscripts that concern Basic Science, Animal Studies, Cadaver Studies, and Experimental Studies. For a full description of these Evidence-Based Medicine ratings, please refer to the Table of Contents or the online Instructions to Authors http://www.springer.com/00266 .

“…Silicone implants have been shown to reliably result in subcutaneous fibrous capsule formation. 30 And our findings demonstrate that this also occurs in the subconjunctival space and that topical ophthalmic application of pirfenidone inhibits the formation of this fibrous capsule. This model of subconjunctival fibrosis represents a technically simple and cost-effective way of inciting fibrosis compared to the implantation of an actual glaucoma shunt and can be used for the initial evaluation other antifibrotic medications.…”

“…3 Silicone have been reported as the most common implanted material in the human body, and plastic surgeons frequently use siliconebased implants as material for breast augmentation and reconstruction. 4 A recent study has shown that pulsed acoustic cellular expression can significantly reduce capsule thickness around silicone implants 5 indicating a new treatment option, but only for a selected patient group. Capsular contracture typically presents within the first year, 6Y8 but studies have also indicated that a correlation exists between time elapsed from surgery and rate of capsular contraction.…”

“…In our previous study, we reported thinner capsules after single ESWT, indicating the feasibility of using a single application to decelerate capsule formation (Reichenberger et al 2014). The aim of the present study was to compare single and multiple ESWT for the prevention and treatment of capsule formation after implantation of silicone devices, ESWT is non-invasive, is easy to use and has few side effects.…”
