Pulsatile venous waveform quality in Fontan circulation-clinical implications, venous assists options and the future

Abstract: Objective: Functionally univentricular heart (FUH) anomalies are the leading cause of death from all structural birth defects. Total cavopulmonary connection (TCPC) is the last stage of the palliative surgical reconstruction with significant late hemodynamic complications requiring high-risk heart transplantation. Alternative therapeutic options for these critically ill patients are crucial. In Phase I, we investigated the effect of pulsatility of venous flow (VF) waveform on the performance of functional and … Show more

“…Published studies of this approach for Fontan patients are rare . Kocyildirim et al are the most recent group to have studied EECP for use in failing Fontan patients. Their study sought to identify the effect of the counter‐pulsatile flow in the TCPC in three Fontan patients.…”

“…Pharmacological and other therapies have variable success [11][12][13][14]. The probability of successful outcome with heart transplantation is low [15][16][17][18][19]. Some have suggested "assisting" the Fontan circulation using an implanted, intravascular, synthetic, externally powered pump [20][21][22][23].…”
