Pulmonary veno-occlusive disease misdiagnosed as idiopathic pulmonary arterial hypertension

Abstract: A 27-yr-old female with a 6-month diagnosis of idiopathic pulmonary arterial hypertension (PAH) confirmed elsewhere was referred to our centre with worsening dyspnoea. On examination, the patient had low systemic oxygen saturation despite high oxygen flow and reduced exercise capacity. Haemodynamics were indicative of severe pre-capillary PAH. Highresolution computed tomography revealed diffuse ground-glass opacity with thickening interlobular septa, and haemosiderin-laden macrophages were identified by broncho… Show more

“…Mimicking of primarily pulmonary diseases, such as recurrent pneumonia, interstitial pneumonitis, lung cancer and pulmonary embolism, has been reported in animal studies, as in many case reports [19,[90][91][92][93]. Mild interstitial fibrosis is thought to originate from the organisation of haemorrhagic oedema in the alveolar walls.…”

“…PVOD represents a difficult-to-diagnose entity, often leading to challenges in discriminating the disease from other forms of PAH [Palazzini and Manes, 2009;Ye et al 2011]. Alternative diagnosis that should be considered and excluded through the diagnostic workup are parenchymal lung diseases, such as interstitial pneumonia, sarcoidosis, or even pneumoconiosis, congestive heart failure, and chronic thromboembolic PH (because of frequent perfusion defects on ventilation-perfusion scan).…”

“…However, when it comes to response to treatment, data is somewhat disappointing. In fact, some treatments to IPAH may actually be deleterious for these conditions if not used cautiously [32][33][34][35].…”
