Pulmonary sequestration presenting with left upper abdominal bloating and marked elevation of serum carbohydrate antigen 19-9: A case report

Abstract: Carbohydrate antigen 19-9 (CA19-9) is widely accepted as a tumor marker for cancers of the biliary, pancreatic and gastrointestinal tracts. Occasionally, CA19-9 is markedly elevated in the serum of patients with benign diseases. Pulmonary sequestration is a rare malformation that is characterized by the presence of lung tissue with abnormal or absent communication with the bronchi, to which blood is supplied by the systemic arteries. The current study presents a 48-year-old male who presented with upper left a… Show more

“…Many studies show that pulmonary sequestration with elevated CA19-9 is common in adults and intralobar type. CA19-9 was positive in bronchial mucosal epithelium and alveolar epithelium in the area of immunohistochemical lesion ( 7 , 9 - 13 ). It was speculated that pulmonary sequestration lesions, especially complicated with aspergillus or other pathogenic infection, promoted bronchial epithelial cells to synthesize and secrete CA19-9 ( 14 ), gathered in the dilated bronchial sac cavity, and entered into the blood through the damaged mucosa of the cyst wall ( 15 ).…”

“…CA 19-9 level is also elevated when the pathway discharging CA 19-9 is obstructed by duct stenosis or stones. Additionally, chronic hepatitis and chronic kidney disease can increase serum CA 19-9 level by hindring CA 19-9 metabolism [2]. CA 19-9 secreted by the bronchiolar epithelia may appear in the serum as a result of airway damage in the lower respiratory tract, such as pulmonary sequestration.…”

“…Pulmonary sequestration accounts for 0.15-6.4% of all congenital pulmonary malformations and receives its blood supply from a systemic artery rather than the pulmonary arterial branch [1][2][3][4][5]. The predominant clinical symptoms of pulmonary sequestration consist of coughing (50%), fever (25%), hemoptysis (22.7%), expectoration (18.2%), chest tightness (15.9%) and thoracic pain (11.4%) [6]. Diagnosis is mainly based on chest CT to find local focal opacity, especially in the left lung (Fig.…”
