Pulmonary Sarcoidosis following Etanercept Treatment

Abstract: Tumour necrosis factor (TNF) is an important cytokine involved in the pathology of a number of inflammatory conditions, and thus blockade with anti-TNF therapies is becoming the cornerstone in managing such diseases. With increasing use, evidence is collected for the association of sarcoid-like granulomatous disease developing after the initiation of anti-TNF-α therapy, with disease reversal after discontinuation.

“…It should be noted that sarcoidosis may involve any organ [3] or it can be limited to the eyes only [12]. The same is true for induced sarcoidosis, which may be limited to 1 organ [4, 7, 8, 9, 10, 11]. For example, it has been reported to present very atypically and involve just the hypopharynx [13].…”

“…TNFα-induced sarcoidosis may not demonstrate all the systemic manifestations of the disease and may involve only 1 organ. Several case reports have been published describing pulmonary [ 7 , 8 , 9 ] or cutaneous [ 10 , 11 ] sarcoidosis following treatment with TNFα antagonists.…”

“…At least 47 cases have occurred. Fortunately, the prognosis is good, with 37 of 47 patients experiencing complete resolution of the event after discontinuation [15,16]. …”

“…Coexistence of sarcoidosis and sacroiliitis has been published in numerous case reports [ 9 ]; however the prevalence of sacroiliitis and spondyloarthritis has not been reported in sarcoidosis. Evidence is collected for the association of sarcoid-like granulomatous disease developing after the initiation of anti-TNF- α therapy, with disease reversal after discontinuation [ 10 ]. Therefore, this study is designed to determine the prevalence of sacroiliitis and spondyloarthritis in patients diagnosed with sarcoidosis and to establish any possible correlation with the findings of the disease.…”
