Pulmonary Mucosa-Associated Lymphoid Tissue Lymphoma in a Patient with Common Variable Immunodeficiency Syndrome

Abstract: Common variable immunodeficiency syndrome (CVID) is a primary immunodeficiency typically presenting with recurrent sinopulmonary infections. Non-Hodgkin’s lymphoma and other secondary cancers are typical late complications of CVID. We report on a patient suffering from CVID with a history of recurrent sinopulmonary infections, interstitial pulmonary changes and hepatic granulomas. Despite treatment with intravenous immunoglobulin followed by a reduction in the number of pulmonary infections, reticular and nodu… Show more

“…Increasing sputum purulence has been clearly associated with bacterial growth in sputum samples (Allegra et al 2005; Blasi et al 2006). Measurement of serum procalcitonin levels have recently been suggested to define AECOPD patients with a higher likelihood of bacterial infection (Reichenberger et al 2001; Christ-Crain et al 2004). Hence, the available data suggest that antimicrobial therapy provides benefit in carefully selected patients with more severe disease and more severe exacerbations.…”

“…Spleen enlargement was also present in our patient, another typical symptom of CVID. Malignant lymphoproliferative diseases are reported as complications of CVID, namely different histological entities of Non-Hodgkin lymphoma (for overview see [12]), lymphoma of the CNS [13], gastrointestinal tract [14], lung [15], T cell lymphoma [16], and also Hodgkin's disease [17,18] develops in some patients. The BCL6 overexpression in the immunohistology is of note since CVID related lymphomas are reported to be associated with mutations and rearrangements of BCL6 [19].…”

“…Biopsies may be necessary for the diagnosis of ILD in PAD and to differentiate pulmonary nodules from malignancies, particularly lymphomas, which may arise in the lungs of these patients. 106 Pulmonary function testing may reveal obstructive pathology, particularly in PAD patients with bronchiectasis, or restrictive lung disease resulting in reductions of forced vital capacity (FVC) and/or diffusing capacity for carbon monoxide (DLCO) as a consequence of ILD. 86, 107 Significant decrease of DLCO may precede decline of FVC in PAD patients with ILD, and therefore should be included in pulmonary function assessment of these patients.…”
