Pulmonary Hypertension in Severe Acute Respiratory Failure

Abstract: We repeatedly assessed pulmonary and systemic hemodynamics in 30 patients undergoing therapy for severe acute respiratory failure of diverse causes. Pulmonary-artery hypertension and elevated pulmonar vascular resistance were observed in all patients after correction of systemic hypoxemia. Increasing pulmonary blood flow by isoproterenol infusion or decreasing pulmonary blood flow by partial bypass of the right side of the heart minimally altered pulmonary-artery pressure. Although neither elevated pulmonary v… Show more

“…121 The finding of fibrosing alveolitis on lung biopsy correlates with an increased mortality from ALI/ARDS. 122 Although fibrosing alveolitis typically develops many days into the course of ALI/ARDS, the molecular mechanisms that determine whether a patient will develop 123 fibrosing alveolitis may be set in motion remarkably early in the course of ALI/ARDS and may be a function of the severity of the initial lung injury. Levels of procollagen III peptide in the alveolar compartment may be elevated very early in the course of ALI/ARDS, even at the time of diagnosis; higher levels are associated with increased mortality.…”

“…Extensive remodelling of the pulmonary vascular bed also takes place. An increase in lung collagen levels can be detected in patients with lung injury of >14 days duration, and continue to increase for the duration of the disease [13].…”

“…In contrast to the systemic vasodilatation, pulmonary hypertension often occurs in the acute phase of endotoxaemia. This may lead to rightsided heart failure, pulmonary oedema, hypoxia and death (Zapol et al, 1977;Demling et al, 1981).…”
