According to epidemiological data on type 2 diabetes (T2DM) represents about 95 % of cases of diabetes diagnosed, with projected to exceed 480 million in 2030. In addition, pulmonary arterial hypertension (PAH) has an estimated prevalence of 30 to 50 cases per million population, affects women more often than men, and PAH and T2DM are medical conditions that presents statistical correlation. The aim of this study was to evaluate the treatment in patients with diabetic that despite age, reported no episodes of hypoglycemia during the case study. Larger studies are needed to assess whether the same results are reproducible in diabetics and also euglycemic patients with PAH. Case 1, SPLM, patient 63-year-old female with a previous diagnosis of T2DM, obesity, hypertension and dyslipidemia forwarded to endocrine evaluation for better glycemic control and weight loss. Case 2, HFC, patient 74-year-old female with a previous diagnosis of T2DM about three years, obesity, hypertension, dyslipidemia osteopenia, vitamin D deficiency and pulmonary hypertension. In the present study, this is the first description in humans that use a iDDP4 may be a treatment option for patients with PAH by the presence of receptor GLP-1R in the lung. It was concluded that in the first case there was weight loss and improved glycemic level. In the second case, both the glucose level of the weight remained. In both cases, using vildagliptin, there was a decrease in systolic pressure of right ventricle pointed out that the relaxation of pulmonary artery has a more beneficial effect would be mediated by NO and activation of vascular potassium channels, further of the effect direct by increasing GLP1, since the IRS1 less inhibited increases the activation of kinases, which in turn increases the stimulus for nitric oxide production in endothelial cells, reducing pulmonary arterial pressure.

The aim of this study was to evaluate the association between thyroid hormone levels, pulmonary hypertension (PH), and pulmonary artery systolic pressure (PASP) in euthyroid patients with coronary artery disease (CAD). A cross-sectional study was conducted in individuals who underwent coronary angiography and were diagnosed as CAD from March 2013 to November 2013. 811 subjects (185 women and 626 men) were included in this study. PASP was measured by transthoracic Doppler echocardiography. 86 patients were diagnosed as PH and had significantly higher free thyroxine (FT4) levels than those without PH. Multiple logistic regression analysis demonstrated an independent association of FT4 levels with PH after adjustment of gender, age, body mass index, systolic blood pressure, left ventricular ejection fraction, hypertension, and medication use of calcium channel blockers, ACE inhibitors, angiotensin II receptor antagonists, and nitrates. Serum-free triiodothyronine (FT3) and thyroid-stimulating hormone (TSH) were not associated with PH. Furthermore, multivariate linear regression analysis showed that FT4 levels emerged as an independent predictor for PASP, while FT3 and TSH levels were not associated with PASP. Our study demonstrated that, in euthyroid patients with CAD, FT4 was an independent risk factor for PH, and FT4 levels were independently associated with PASP.

REzumatDatele existente în literatura de specialitate internațională au relevat o prevalență semnificativ crescută a afecțiunilor tiroidiene, în principal a celor autoimune, la pacienții cu hipertensiune arterială pulmonară (HAP) și, invers, o prevalență crescută a HAP la pacienții cu hipertiroidie sau tiroidită autoimună Hashimoto cu eutiroidie. Studiile efectuate până în prezent nu au identificat mecanismul patogenetic al apariției HAP la pacienții cu afecțiuni tiroidiene. Normalizarea promptă a HAP apărută în contextul hipertiroidismului, după tratamentul adecvat al hipertiroidismului și obținerea eutiroidiei, a fost dovedită după multe studii clinice. Există date limitate în literatura de specialitate despre evaluarea comparativă a eficacității diferitelor metode terapeutice ale hipertiroidiei (tratamentul cu antitiroidiene de sinteză, tratamentul chirurgical și tratamentul cu iod radioactiv) în reducerea presiunii pulmonare.Cuvinte cheie: hipertensiune arterială pulmonară, hipertiroidism, tiroidită autoimună aBStRaCt The existent data in the international speciality literature revealed a significantly high prevalence of thyroid disorders, mainly of those autoimmune, in patients with pulmonary arterial hypertension (PAH), and, conversely, an high prevalence of PAH in patients with hyperthyroidism or Hashimoto autoimmune thyroiditis with euthyroidism. The studies carried out until the present did not identify the pathogenetic mechanism of the occurrence of PAH in patients with thyroid disorders. The prompt normalization of PAH appeared in context of the hyperthyroidism, after adequate treatment of the hyperthyroidism and after obtaining euthyroidism, has been proven after many clinical trials. There are limited data in the speciality literature about comparative assessment of the effectiveness of the different therapeutic methods of hyperthyroidism (the treatment with antithyroid drugs, surgical treatment and radioactive iodine treatment) in reduction of pulmonary pressure.
