Pulmonary Function in Chronic Severe Anaemia

Abstract: 1. The process of pulmonary ventilation and gas exchange was investigated in twenty-three young patients with chronic severe anaemia, before and after its correction. 2. Various lung volumes, pulmonary mechanics, minute ventilation, oxygen consumption and carbon dioxide production were found to be normal in anaemic patients. 3. There was a mild respiratory alkalosis in anaemia. The arterial oxygen tension was lowered because of a marked widening of the alveolar-arterial oxygen ten… Show more

“…The central or overall respon siveness to increments in C 0 2 is, however, normally preserved. These observations are similar to the findings in permanent high al titude natives [5,7] or patients with cyanot ic congenital heart disease [8], The degree of arterial hypoxia in subjects with anaemia is only minimal [2], although tissue hypoxia may be considerably more pronounced. Moreover, it is not present from birth and acquired for relatively shorter period of time during adult life.…”

“…Cerebral hypoxia in itself may have a depressant ef fect on the respiratory centres. Because of these conflicting effects, the resting ventila tion in anaemia has generally been found to be normal [2]. Moreover, the ventilatory re sponse to hypercapnia and hypoxia also has been reported to be normal in chronically anaemic rats [1].…”

“…Thus the explanation for the results before treatment remains unclear. The findings may be contrasted with those for patients with either more-marked irondeficiency anaemia (Rankin et al, 1961 ;Guleria et al, 1971) or sickle-cell anaemia (Miller & Serjeant, 1971 ;Femi-Pearse, Gazioglu & Yu, 1970) in whom the Dm/Vc ratio was decreased on account of low values for Dm which were in some instances compensated for by high values for Vc. This lability of the ratio emphasizes the desirability of making a full allowance for variation in haemoglobin concentration by measurement of Dm and Vc wherever possible.…”

“…The dependence of this index on haemoglobin concentration has been demonstrated previously in patients with chronic anaemia (Guleria, Pande, Markose, Gupta & Jain, 1971;Oinakara, Blumenthal, Johnston, Kauffman & Solnick, 1970) and polycythaemia (Herbert, Weill, Stuckey, Urner, Gonzales & Ziskand, 1965;Burgess & Bishop, 1963).…”
