Pulmonary Function Abnormalities in HIV-Infected Patients during the Current Antiretroviral Therapy Era

Abstract: Rationale: Before the introduction of combination antiretroviral (ARV) therapy, patients infected with HIV had an increased prevalence of respiratory symptoms and lung function abnormalities. The prevalence and exact phenotype of pulmonary abnormalities in the current era are unknown. In addition, these abnormalities may be underdiagnosed. Objectives: Our objective was to determine the current burden of respiratory symptoms, pulmonary function abnormalities, and associated risk factors in individuals infected … Show more

“…There have been three recent prospective studies that examined respiratory symptoms and measured pulmonary function in the era of combination ART (13)(14)(15). The first study performed spirometry in 234 HIV-infected outpatients without a history of acute respiratory disease or asthma (13).…”

“…Risk for premature comorbidities, such as cardiovascular disease, renal, and liver disease, has been associated with residual inflammation and immunodeficiency despite treatment with ART (20). A possible association of ART with airflow obstruction has been suggested (21,22), although ART has not been found to have direct toxic effects on the lungs. The consequences of persistently increased inflammation and the effects of ART on the pathogenesis of lung complications in HIV-infected persons are poorly understood.…”

“…A recent observational study documented respiratory symptoms and functional abnormalities in 47.3% and 64.1% of study participants respectively. Irreversible airways obstruction compatible with COPD, independently associated with pack years smoked, intravenous drug use and the use of anti -retroviral therapy was found in 21% of study participants 267 . In patients with obstructive airways disease, HIV infection appears to confer a higher risk of moderate but not severe dyspnoea 268 .…”
