Pulmonary cryptococcal infection presenting with multiple lung nodules

Abstract: Pulmonary infections from the environmental fungus Cryptococcus gattii (C. gattii) are notable for cryptococcomas, which are usually solitary and can be very large. As with infections with Cryptococcus neoformans (C. neoformans) patients can have concomitant cryptococcal meningitis; however, unlike for C. neoformans, infections with C. gattii often occur in immunocompetent patients. The highest published incidence of C. gattii infection has been in the Indigenous Australian population of Arnhem Land in the tro… Show more

“…Brain cryptococcoma can cause obstructive hydrocephalus via a mass effect, 23 and our data showed hydrocephalus occurred in 11.8% of CGM patients and 18.8% of CNM patients, which were less common than those in an Australian‐population‐based study, where 32%–35.3% of CGM patients experienced hydrocephalus 24 . Furthermore, we observed that meningeal enhancement and pulmonary cryptococcomas occurred more frequently in CGM patients, which consistent with current report 25 . However, the incidence rates of lung cryptococcomas (71.4% in VGI and 20% in VGII) in CGM patients here differ somewhat from those in Australia (47%), 14 British Columbia (66.4% in VGII), 12 and the United States (29–42% in VGII and 61% in VGI or VGIII) 20,22 .…”

“…A formação de nódulos e massas de infiltração pulmonar estão relacionados com sítios de latência e, consequentemente de disseminação das leveduras diante de uma predisposição do hospedeiro, como vulnerabilidade imunológica. No entanto, em pacientes imunocompetentes, apesar de ser associada com achados radiológicos atípicos e/ou agressivos, os sintomas desaparecem gradualmente após o tratamento (XIE et al, 2015;BASNAYAKE et al, 2018;SUWATANAPONGCHED et al, 2013).…”
