Pulmonary artery perforation and coronary air embolism—two fatal outcomes in percutaneous left atrial appendage occlusion

Abstract: Percutaneous left atrial appendage (LAA) closure is a routinely performed method to reduce the risk of stroke in patients suffering from atrial fibrillation, when an oral anticoagulation is no longer indicated due to relevant bleeding complications. “…Unfortunately, pericardial tamponade occurred, which was caused by the barbs of the device eroding into the adjacent PA. PA perforation is a dreaded complication, and previous cases reported similar complications caused by an Amplatzer Amulet or Amplatzer Cardiac Plug. 3,4 Unstable hemodynamics suddenly occurred several hours after the procedures. Although emergency resuscitation was performed, the two patients still died, which may be due to a greater penetration depth (1.5 mm for the Amulet hooks, 0.5 mm for the WATCHMAN barbs) from the Amplatzer Amulet or Amplatzer Cardiac Plug than from the WATCHMAN device.…”
