Pulmonary Arterial Hypertension Induced by Distention of the Main Pulmonary Artery in Conscious Newborn, Young, and Adult Sheep

Abstract: SummaryDistention of the main pulmonary artery by balloon inflation in sheep results in presumably reflex elevation of pulmonary arterial pressure and resistance distal to the balloon. This response to main pulmonary artery distention is significantly greater in newborn lambs than in older lambs or adult sheep. In several of the newborn lambs, pulmonary artery pressure was raised to suprasystemic levels. Further, in some of the newborn animals, these increases in pulmonary artery pressure and resistance were s… Show more

“…First, in our experiments, the shunt was central and undoubtedly caused some distension of the pulmonary artery. Distension of the pulmonary artery has been shown to increase vascular tone in downstream vessels and it is possible that such an increase in tone occurred in our experiments (18)(19)(20). The lack of a significant decrease in pulmonary vascular resistance with increasing blood flow in our experiments supports this possibility.…”

“…nia, persistent fetal circulation often occurs, making postoperative management difficult. Postoperatively, after the relief of lung compression, a sudden increase in lung volumes and distention of the main pulmonary artery may produce vasoconstriction of the distal pulmonary arterial bed [10]. In contrast, severe vasoconstriction is not expected to occur in tricuspid valve disease because the small pulmonary arteries have relatively thin smooth muscle.…”

“…Pulmonary artery baroreceptor activation reflexively causes pulmonary vasoconstriction [134,135], probably via adrenergic nerves [136]. This provides the rationale for PADN as a way to reverse pulmonary vasoconstriction.…”
