Clinical Patterns of Traditional Chinese Medicine for Ischemic Heart Disease Treatment: A Population-Based Cohort Study

Background and objectives: Traditional Chinese medicines (TCMs) are widely prescribed to relieve ischemic heart disease (IHD); however, no cohort studies have been conducted on the use of TCMs for patients with IHD. The aim of the study was to analyze TCM prescription patterns for patients with IHD. Materials and Methods: The retrospective population-based study employed a randomly sampled cohort of 4317 subjects who visited TCM clinics. Data were obtained from the National Health Insurance Research Database (NHIRD) of Taiwan for the period covering 2000 to 2017. Data analysis focused on the top ten most commonly prescribed formulae and single TCMs. We also examined the most common two- and three-drug combinations of TCM in single prescriptions. Demographic characteristics included age and sex distributions. Analysis was performed on 22,441 prescriptions. Results: The majority of TCM patients were male (53.6%) and over 50 years of age (65.1%). Zhi-Gan-Cao-Tang (24.76%) was the most frequently prescribed formulae, and Danshen (28.89%) was the most frequently prescribed single TCM for the treatment of IHD. The most common two- and three-drug TCM combinations were Xue-Fu-Zhu-Yu-Tang and Danshen” (7.51%) and “Zhi-Gan-Cao-Tang, Yang-Xin-Tang, and Gua-Lou-Xie-Bai-Ban-Xia-Tang” (2.79%). Conclusions: Our results suggest that most of the frequently prescribed TCMs for IHD were Qi toning agents that deal with cardiovascular disease through the promotion of blood circulation. The widespread use of these drugs warrants large-scale, randomized clinical trials to investigate their effectiveness and safety.

Pueraria lobata (Willd.) Ohwi is a medicinal and edible homologous plant with a long history in China. Puerarin, the main component isolated from the root of Pueraria lobata, possesses a wide range of pharmacological properties. Daidzein and glucuronides are the main metabolites of puerarin and are excreted in the urine and feces. As active substrates of P-gp, multidrug resistance-associated protein and multiple metabolic enzymes, the pharmacokinetics of puerarin can be influenced by different pathological conditions and drug-drug interactions. Due to the poor water-solubility and liposolubility, the applications of puerarin are limited. So far, only puerarin injections and eye drops are on the market. Recent years, researches on improving the bioavailability of puerarin are developing rapidly, various nanotechnologies and preparation technologies including microemulsions and SMEDDS, dendrimers, nanoparticles and nanocrystals have been researched to improve the bioavailability of puerarin. In order to achieve biocompatibility and desired activity, more effective quality evaluations of nanocarriers are required. In this review, we summarize the pharmacokinetics and drug delivery systems of puerarin up to date.

Warfarin merupakan obat antikoagulan oral yang digunakan untuk menangani penyakit terkait dengan tromboembolisme. Warfarin memiliki indeks terapi yang sempit serta memiliki kecenderungan berinteraksi dengan obat maupun herba sehingga membutuhkan pemantauan yang cermat pada efek farmakologi yang ditimbulkan. Penentuan pemberian dosis serta pengamatan efek dari interaksi warfarin dengan obat atau herba salah satunya dapat diketahui dengan menganalisis konsentrasi warfarin dalam plasma darah. Artikel ini merupakan kajian pustaka yang bertujuan untuk memberikan tinjauan mengenai metode yang digunakan untuk menganalisis konsentrasi warfarin dalam plasma darah serta teknik preparasi sampelnya. Metode yang digunakan dalam review artikel ini menggunakan penelusuran melalui basis data PubMed®. Hasil yang didapatkan bahwa metode yang umum digunakan untuk determinasi warfarin dalam plasma darah yakni high-performance liquid chromatography (HPLC) serta ultra-HPLC dengan detektor ultraviolet (UV), fluoresensi (FLD), diode array (DAD), maupun spektroskopi massa (MS). Metode lainnya yaitu spektrofotometri UV/Vis dan spektrofotometri fluoresensi serta elektroforesis. Dari metode tersebut, metode HPLC dengan detektor spektroskopi massa paling banyak digunakan untuk menganalisis warfarin serta teknik preparasi sampel yang umum digunakan yakni preparasi sampel yang umum digunakan yakni presipitasi protein menggunakan asetonitril sebagai protein presipitan serta penambahan asam untuk meningkatkan nilai recovery warfarin dari plasma darah
