IntroductionVirtual reality (VR) has been shown to have a certain influence on the psychological health of intensive care unit (ICU) patients. However, its specific effects—particularly on psychological health problems, such as psychological well-being, quality of life (QOL) and patient satisfaction—remain unclear.Method and analysisThis study follows the Preferred Reporting Items for Systematic Review and Meta-analysis Protocols guidelines. Electronic data search is carried out on PubMed, Web of Science, CINAHL, EBSCO, EMBASE, Cochrane Library, PsycINFO, China National Knowledge Infrastructure, Wan Fang, VIP and Chinese Biology Medicine Database. The inclusion criteria follow the PICO principle, wherein ICU patients who have been hospitalised for 24 hours or more are included. Studies using VR-based interventions to improve the psychological health of ICU patients, compared with waitlist controls or traditional therapy groups; outcome assessments containing psychological well-being, QOL and patient satisfaction; and those designed as randomised controlled trials (RCTs) and quasi-experimental research are included. Search time is from inception of each database to July 2023. No language restriction is considered. Studies for inclusion are screened by two independent reviewers for data extraction. Any dispute is resolved through discussion. Unresolved disputes are decided on by consulting a third author. For the risk of bias assessment in RCTs and non-RCTs, the Cochrane risk-of-bias tool for randomised trials and risk of bias in non-randomised studies of interventions tool are used, respectively. For meta-analysis, RevMan V.5.3 is used.Ethics and disseminationThis protocol study does not include clinical research and thus does not require ethical approval. Research findings will be released in a peer-reviewed journal.PROSPERO registration numberCRD42023400428.

Dyspnea is a subjective sensation often described as a feeling of respiratory effort, tightness, or air hunger. The underlying mechanisms of this symptom are multifaceted and involve factors such as respiratory centers, cardiovascular system, airways, neuromuscular components, and metabolic factors, although not fully elucidated. The classical theory of imbalance between inspiratory neural drive (IND) and the simultaneous dynamic responses of the respiratory system posits that the disruption of a normal and harmonious relationship fundamentally shapes the expression of respiratory discomfort. Assessment and comprehensive treatment of dyspnea are crucial for patient rehabilitation, including subjective self-reporting and objective clinical measurements. Non-pharmacological interventions, such as pulmonary rehabilitation, fan therapy, exercise, chest wall vibration, virtual reality technology, traditional Chinese medicine (acupuncture and acupressure), and yoga, have shown promise in alleviating dyspnea symptoms. Additionally, oxygen therapy, has demonstrated short-term benefits for patients with pre-hospital respiratory distress and hypoxemia. This review provides a comprehensive overview of dyspnea, emphasizing the importance of a multifaceted approach for its assessment and management, with a focus on non-pharmacological interventions that contribute to enhanced patient outcomes and quality of life.

BACKGROUND Digital technologies and emerging innovations are increasingly being used to strengthen national health systems. Based on scientific recommendations, Music Care©, a web app-based personalized music intervention has been developed to provide a collection of culturally diverse original pieces composed and arranged following standardized specifications (U-Sequence). The Music Care app has been utilized in a broad variety of medical conditions and procedures to reduce anxiety and pain in patients with various diseases. OBJECTIVE The aim of this report is to provide a review of the available evidence on the efficacy of Music Care app for the management of pain and anxiety. The specific medical context in which Music Care app has been tested is also examined to identify potential new applications and areas requiring more research. METHODS Original English and French language randomized controlled trials (RCT) using the Music Care app were included and reviewed to summarize evidence on the effectiveness of a music intervention program in medicine. Ongoing RCT were also reported. RESULTS A total of 17 RCTs were included. The majority of studies focused on chronic and acute pain conditions. Compared to the control interventions, Music Care was effective in reducing pain and anxiety in most medical contexts examined, including chronic pain, Alzheimer, and during surgeries or procedures. Significant improvements have also been found on cardio-respiratory activity, depressive symptoms, stress and burnout levels. CONCLUSIONS The Music Care app has proven effective in reducing pain, anxiety, depressive symptoms and stress and burnout symptoms in various medical conditions and procedures. Growing evidence suggests that the Music Care app could provide a useful digital, low-cost, portable and easy-to-implement tool as an adjuvant therapy in a broad scope of clinical situations.
