Validation of the 36‐item and 12‐item self‐report World Health Organization Disability Assessment Schedule II (WHODAS‐II) in individuals with autism spectrum disorder

The World Health Organization Disability Assessment Schedule II (WHODAS‐II) is one of the most widely used generic assessments for measuring disability levels in both clinical and nonclinical populations, with sound psychometrics that is also aligned with the International Classification of Functioning framework. However, its psychometric properties have not been explored extensively in individuals with autism spectrum disorder (ASD). This study examined the psychometric properties of the 36‐item and 12‐item Self‐Report WHODAS‐II from 109 individuals diagnosed with ASD and without intellectual disability (IQ ≥ 70). Participants were consecutively recruited from the Brain and Mind Centre in New South Wales, Australia. The WHODAS‐II showed adequate internal consistency for all domain scores (α = 0.78–0.97 for 36‐item) and for the summary scale (α = 0.95 for 36‐item; 0.86 for 12‐item). All items also exhibited satisfactory correlations with their respective domain (r = 0.39–0.94 for 36‐item) and summary scores (r = 0.42–0.71 for 36‐item; 0.42–0.67 for 12‐item), except item 4.5 “sexual activity” from the 36‐item WHODAS‐II (r = 0.19). Concurrent validity was shown by moderate correlations between similar constructs across the WHODAS‐II and the World Health Organization Quality of Life BREF (Ps < 0.05). The second‐order 7‐factor model showed the best fit for the 36‐item WHODAS‐II, while the second‐order 6‐factor model demonstrated an acceptable fit for the 12‐item WHODAS‐II. The model fit could be improved with some modifications. The Schmid‐Leiman transformation further confirmed the appropriateness of the second‐order factor structure. Overall, the results indicated that the WHODAS‐II is a viable generic self‐report measure for disability in autistic individuals without ID. Lay Summary The majority of autistic people have a disability with a profound or severe limitation in their core activities. However, there is currently limited research identifying reliable and valid self‐report measures for disability in the autistic population. Our results suggest that the WHODAS‐II is a viable generic self‐report measure for disability in autistic individuals.

Background—Mental disorders have been found to affect quality of life (QOL) in patients. The current study aimed to determine QOL among first episode psychosis (FEP) patients and explore its associations with sociodemographic as well as clinical factors. Methods—Data for this study were collected as a part of an Early Psychosis Intervention Program (EPIP)-Smoking and Alcohol use survey. At baseline, 280 outpatients aged 15–40 years old diagnosed with FEP, with no prior or minimal treatment, no history of medical or neurological disorder, and no history of substance abuse, were recruited. Sociodemographic details, diagnosis, length of duration of untreated psychosis (DUP), and World Health Organization Quality of Life assessment—abbreviated version (WHOQOL-BREF) scores were obtained. Results—After adjusting for all covariates, older age (p = 0.036), females, and participants diagnosed with brief psychotic disorder (p = 0.04) were associated positively, whereas separated/divorced participants, those with lower education, unemployed (p = 0.01), and longer DUP were seen to be negatively associated with different domains of QOL. Conclusion—Higher WHOQOL-BREF scores denote better QOL. Overall, female participants as compared to male participants and those diagnosed with brief psychotic disorder in this sample reported better QOL.

Background: The amount of waste generated has been increasing over the years, while the ability to dispose of it does not meet the needs at waste treatment plants (WTP), resulting in negative consequences on the natural environment and health of the residents living near these plants; these effects can significantly degrade their quality of life (QoL). Although previous studies on the impact of solid waste and wastewater treatment plants have focused on the health problems suffered by nearby residents, the impact of these plants has scarcely been discussed in the context of mental health, social relationships, and the environment. The objectives of this study are to evaluate the QoL as a comprehensive indicator of the impact the presence of a WTP has on nearby residents, and determine the factors that influence their QoL. Methods: A cross-sectional descriptive study was conducted involving 801 subjects, aged 18 and above, who live near the WTP of Hue city, Thua Thien Hue province, Vietnam. The QoL of the subjects was quantitatively assessed using the World Health Organization's (WHO) QoL assessment scale (WHOQOL-BREF). The general, health, and environmental factors influencing QoL were identified using bivariate and multivariate logistic regression analyses. Results: It was found that only 22.6% of the subjects have a good QoL. In particular, the proportions for good psychological health (6.9%) and environment (13.6%) were low, which may indicate an influence of the WTP. Significant factors that degraded the residents’ QoL are: a lower educational level defined by not graduating from high school (OR=2.78; 1.09−7.06), poor health status (OR=2.50; 95% CI: 1.56−4.01), dissatisfaction with water quality (OR=2.41; 95% CI: 1.10−5.25), and unacceptance of the WTP presence (OR=1.70; 95% CI: 1.11−2.60). Moreover, it was found that subjects living within 2 km of the plant had dermatological syndromes and digestive disorders more frequently than those who lived farther away from the plant. They also reported more complaints about water, air, and soil quality, which were probably due to the operation of the WTP. Conclusions: Burying and disposing of solid waste at the WTP could lead to a degradation of the surrounding water and soil environments, while its collection and transportation is believed to cause odor and dust. The efforts of responsible authorities to strictly supervise and inspect these activities at the WTP are essential, not only to protect the surrounding environment but also to improve the QoL of those who live near these plants.
