Aim To evaluate the effects of a self‐care promoting problem‐based learning programme for people with rheumatic diseases in terms of health‐related quality of life, empowerment, and self‐care ability. Background Individuals with rheumatoid arthritis express a great need for education and support in adapting to the disease, but the average qualities of studies about patient education interventions are not high. There is no evidence of long‐term benefits of patient education. Design Randomized controlled trial. Methods A randomized controlled design was selected with test at baseline, 1‐week and 6‐month post‐interventions after completed the 1‐year programme. The tests consisted of validity and reliability tested instruments. The participants were randomly assigned in spring 2009 to either the experimental group (n = 54) or the control group (n = 148). The programme was running alongside the standard care the participants received at a rheumatology unit. Parametric and non‐parametric tests were used in the analyses. Results The participants in the experimental group had statistically significant stronger empowerment after participation in the self‐care promoting problem‐based learning programme compared with the control group, at the 6‐month post‐intervention. Approximately, two‐thirds of the participants in the experimental group stated that they had implemented lifestyle changes due to the programme. Conclusion The self‐care promoting problem‐based learning programme enabled people with rheumatic diseases to improve their empowerment compared with the control group. It is important to continue to develop problem‐based learning in patient education to find the very best way to use this pedagogical method in rheumatology care.

Measuring Empowerment in Patients with Chronic Disease: Psychometric Qualities of the Portuguese Rheumatic Disease Empowerment Scale

Empowerment is usually defined as a process of promoting the sense of competence and control as a means of capacitating individuals to improve their life conditions. The aim of this study was to describe the psychometric qualities of the Portuguese Rheumatic Disease Empowerment Scale (P-RES-8), an 8-item measure of empowerment for patients with rheumatic disease. The study enrolled 81 patients with rheumatic diseases. Participants filled in the P-RES-8, the Portuguese validated version of the Medical Outcomes Survey Short Form-36 (MOS SF-36), and a questionnaire including sociodemographic data and disease-related variables. To evaluate the dimensionality of the P-RES-8 scale, an exploratory factor analysis was conducted, and its reliability was assessed by Cronbach’s α. The validity of this measure was assessed by analyzing the correlations between empowerment, quality of life, and other relevant variables (education level and illness duration). The P-RES-8 proved to be a reliable 1-dimensional measure of empowerment in patients with rheumatic disease (Cronbach’s α = 0.94). Empowerment was positively related to quality of life. Particularly, the findings showed positive associations between empowerment, assessed by the P-RES-8, and each of the 8 domains of the MOS SF-36. The associations were stronger for mental health and general health perception. The results also revealed that empowerment is positively associated with a higher education and more years after the diagnosis. The findings provide support for the adequacy of the P-RES-8 as a measure of empowerment in patients with rheumatic disease. Moreover, empowerment showed to be an important variable in the assessment of patients’ quality of life.

Development and Validation of the Empowerment Scale for Woman with Breast Cancer

연구의 필요성유방암은 유방암여성 임파워먼트 측정도구 개발 및 타당화Purpose: The purpose of this study was to develop a scale to evaluate empowerment in woman with breast cancer and to examine the validity and reliability of the scale. Methods: The development process for the initial items included a literature review, interviews, and construction of a conceptual framework. The identified items were evaluated for content validity by experts, resulting in 3 factors and 48 preliminary items. Participants were 319 women with breast cancer recruited to test reliability and validity of the preliminary scale. Data were analyzed using item analysis, confirmatory factor analysis, criterion related validity, internal consistency and test-retest reliability. Results: The final scale consisted of 30 items and 3 factors. Factors, including 'intrapersonal factor' (14 items), 'interactional factor' (8 items), and 'behavioral factor' (8 items), were drawn up after confirmatory factor analysis. Goodness of fit of the final research model was very appropriate as shown by χ 2 /df=1.86, TLI=.90, CFI=.92, SRMR=.06, and RMSEA=.05. Criterion validity was evaluated by total correlation with the Cancer Empowerment Questionnaire .78. Cronbach's alpha for total items was .93 and test-retest reliability was .69. Conclusion: Findings from this study indicate that the scale can be used in the development of nursing interventions to promote the empowerment of women having breast cancer. If the original work is properly cited and retained without any modification or reproduction, it can be used and re-distributed in any format and medium.
