Psychometric properties of the EQ-5D-5L in patients with hip or knee osteoarthritis: reliability, validity and responsiveness

Abstract: The results support the reliability, validity and responsiveness of the EQ-5D-5L, overcoming the limitations of the EQ-5D-3L in these patients. Therefore, the EQ-5D-5L could be very useful as an outcome measure, at least in patients with hip or knee OA.

“…Interestingly, the index value and EQ VAS of patients whose response to the GRCQ was "improvement" increased signi cantly, while the different result was discovered in "about the same." Bilbao and colleagues revealed similar results among patients who underwent surgery for hip or knee osteoarthritis, that is the mean change of the EQ-5D-5L score was positive in "improved" group [28]. Patients' perceived health changes, as measured with the GRCQ, were consistent with EQ-5D-5L score changes; even though the GRCQ has only one question and the EQ-5D-5L is a multi-dimensional, multi-attribute questionnaire.…”

“…Speci cally, the MCID of index value and EQ visual analog scale (EQ VAS) in patients with chronic obstructive pulmonary disease undergoing pulmonary rehabilitation were 0.051 and 6.9, respectively [25]; while the MCID of index value in patients with type 2 diabetes and elderly people with hypertension were 0.043 and 0.072, respectively [26,27]. In addition, a study evaluating the relationship between MCID and MDC in patients undergoing hip or knee replacement showed that, when the MCID of the index value was 0.32, it could be distinguished from measurement errors even at the individual level [28]. All these studies inform how the EQ-5D-5L can be better applied in clinical settings; however, to our knowledge, no studies have used the EQ-5D-5L to estimate responsiveness and MCID in surgically treated patients with CIN, nor have they analyzed the relationship between MCID and MDC.…”

“…Our trial found an improvement in quality of life assessed with EQ-5D-5L in the intervention group of 0.10 points at T1 and 0.09 points at T2 from baseline. Previous investigations found that a change of 0.03 points was related to a change on the global perceived effect of a treatment in patients with chronic low-back pain [40] and that minimal clinically important difference (MCID) for improvement was 0.07 points in patients with osteoarthritis [41]. Previous studies showed an association between quality of life and physical performance, patients with lower scores in the SPPB also reported worse results in the EQ-5D-5L index value [42].…”

“…Finally, psychometric properties of the EQ-5D-5L showed a minimal floor and ceiling effect (< 3%), a Cronbach's α of 0.86 and a strong correlation from 0.688 to 0.782 with pain and function scores (Bilbao et al, 2018).…”
