In the future, the rehabilitation of adults with hearing loss is likely to involve modern information technology. It is therefore important to determine whether and to what extent adults with hearing losses use the internet. When evaluating hearing rehabilitation, it is reasonable to use self-report questionnaires as outcome measures. The questionnaires used in audiological research are developed and validated for paper-and-pencil administration. Standardized questionnaires used in the audiological context should also be evaluated in an online administration format. Using the internet in the audiological rehabilitation process might be a cost-effective way to include additional rehabilitation components by guiding hearing-aid users on topics such as communication strategies, hearing tactics and how to handle hearing aids. The development of online rehabilitation programs might foster behavioral changes that will positively affect hearing-aid users.Four studies were conducted with a total of 338 participating hearing-aid users. In the first study, the participants' amount of internet use was investigated. In the second study, the administration format (online vs. paper-and-pencil) of four standardized questionnaires used in hearing research and clinics were evaluated. Finally, two randomized controlled trials were performed to evaluate the efficacy of an online rehabilitation program that included professional guidance by an audiologist. The program lasted over five weeks and was designed for adult, experienced hearing-aid users. The effects of the online programs were compared with the effects of a control group. In the first randomized controlled trial, the participants of the control group participated in an online discussion forum without any professional contact (Study III). In the second randomized controlled trial, the control group participants were informed that the rehabilitation program was full and they were transferred to a waiting list pending treatment (Study IV).The results in the first study showed that 60% of the participating hearing-aid users used computers and the internet. The internet use level was at least at the same level for people with hearing loss as for the general age-matched population in Sweden. In the second study a significant main effect of questionnaire format was found for one questionnaire, which evaluated the participants' activity limitations and participation restrictions; the participants had higher scores on the online format than on the paper format. Other than that, no interaction effects for format were found for the other three questionnaires. In the first randomized controlled trial, after the five-week online rehabilitation program, both groups of participants showed significant improvements and the effects were maintained in domains of activity limitation and participation restriction at the six-month follow-up. Hearing impairment interferes with speech communication. Hearing aids are designed to provide amplification for individuals with poor auditory sensitivity. Signal processing algorithms are implemented in hearing aids to further improve speech understanding and to reduce listening effort in noise. When listening effort is reduced, fewer resources are assumed to be needed for speech perception and more resources would thus be freed for cognitive processing of speech. However, this effect has only been reported in young adults with normal hearing, not in hearing aid users. Cognitive abilities, which vary between individuals, have been shown to influence the ability to benefit from hearing aids. Specifically, it is not yet known how individual differences in cognitive abilities interact with signal processing to reduce listening effort. Also, the relationship between cognition and aided speech recognition performance, as a function of acclimatization processes in new hearing aid users, has not been studied previously.This thesis investigated the importance of cognition for speech understanding in experienced and new hearing aid users. Four studies were carried out. In the first three studies (reported in Papers 1 to 3), experienced hearing aid users were tested and the aims were 1) to develop a cognitive test, called the Sentence-final Word Identification and Recall (SWIR) test, to measure the effects of a noise reduction algorithm on processing of highly intelligible speech (everyday sentences). SWIR performance is argued to reflect the amount of remaining resources upon successful speech perception; 2) to investigate, using the SWIR test, whether hearing aid signal processing would affect memory for heard speech; 3) to test whether the effects of signal processing on the ability to recall speech would interact with background noise and individual differences in working memory capacity; and 4) to explore the potential clinical application of the SWIR test by examining the relationship between SWIR performance and self-reported hearing aid outcome. In the fourth study (reported in Paper 4), the aim was 5) to examine the relationship between cognition and speech recognition in noise in new users using various models of hearing aids over the first six months of hearing aid use.Results of the studies reported in Papers 1 and 3 demonstrated that, for experienced users, noise impairs the ability to recall intelligible speech heard in noise. Noise reduction freed up cognitive resources and alleviated the negative impact of noise on memory when speech stimuli were presented in background noise consisting of speech babble spoken in the listener's native language but not in a foreign language. The possible underlying mechanisms are that noise reduction facilitates auditory stream segregation between target and irrelevant speech and reduces the attention captured by the linguistic information in irrelevant speech. Adults with hearing impairment do not always seek help for their hearing or adopt hearing aids and some people who adopt hearing aids do not use or gain benefit from them.Psychosocial factors that influence help-seeking, hearing aid adoption and outcomes illustrate the importance of addressing client experiences and motivation. Although models of health behaviour have been recommended to understand client behaviour, motivation has not been explored in detail. To investigate motivation in hearing rehabilitation, the research in this thesis applied self-determination theory (SDT) as the guiding theoretical framework.The research comprised three quantitative studies and one qualitative study, which together followed an explanatory sequential mixed methods design. The thesis research aims were to (1) identify motivational characteristics of first time hearing help-seekers and determine relationships between these characteristics and the decision whether or not to adopt hearing aids; (2) For the quantitative studies, a total of 253 participants completed a series of questionnaires adapted from SDT. In the first study, associations between autonomous and controlled motivation, personal and audiometric variables and hearing aid adoption were explored with multivariate regression analysis. Three factors were positively associated with increased hearing aid adoption: autonomous motivation, perceived hearing difficulty and poorer hearing. Controlled motivation was not associated with hearing aid adoption.The second study reported outcomes for 216 of the 253 participants with respect to autonomy support. Multivariate regression models were formed that examined associations between autonomous and controlled motivation, personal and audiometric variables, autonomy support and hearing aid fitting outcomes. Autonomy support was not associated with increased hearing aid adoption, but was associated with three outcomes: increased perceived competence for hearing aids, reduced activity limitations and iii increased hearing aid satisfaction. Autonomous motivation was positively associated with one outcome, hearing aid satisfaction.The patterns of effect of autonomous and controlled motivation on hearing aid adoption and hearing aid fitting outcomes prompted the third quantitative study, which sought to examine personal characteristics associated with motivation. Multivariate regression indicated that, in the 253 participants, those who were younger, wanted hearing aids more and reported greater hearing difficulty also reported higher autonomous motivation.Participants with higher controlled motivation were more often referred by others and wanted hearing aids more than those reporting lower controlled motivation.Together, the three quantitative studies revealed a variety of motivational processes that underlie behaviours and actions of participants. Practitioners may benefit from evaluating motivation in order to better understand how help-seekers engage with hearing services
