Purpose: Lymphedema is an important situation that causes physical and psychological lifethreatening problems. This study researches kinesiophobia in lower extremity patients and scrutinizes the relationship between kinesiophobia, physical performance and balance. Methods:This cross-sectional controlled study included 40 individuals diagnosed with lower extremity lymphedema with a mean age of 42.58±10.30 years and 31 healthy individuals with a mean age of 40.65 ± 9.53 years. The evaluation of patients with unilateral lymphedema without cognitive and visual problems and those without orthopedic and neurological disorders that would prevent walking and those without heart disease and hypertension was carried out between November 2018 and March 2019. Static balance was evaluated with standing on One leg Balance Test, fear of movement was evaluated with Tampa Scale Kinesiophobia and functional levels were evaluated with Timed Up and Go test.Results. Static balance and physical performance of lower extremity lymphedema patients were different from healthy individuals (p<0.05). Static balance and physical performance were lower in these patients than in healthy individuals. Patients with lower extremity lymphedema had a severe fear of movement (37 and above). There was a significant moderate correlation between kinesiophobia and balance (r= -0.536 p= 0.001), kinesiophobia and physical performance (r= 0.522 p= 0.001) in lower extremity lymphedema patients. And this correlation was more in healthy individuals.Conclusions: High fear of movement and decreased physical performance and balance were found in patients with lower extremity lymphedema. Early diagnosis and treatment of complications due to lymphedema is important for the clinical results of this patient group.

Backgrounds Gynaecological cancer survivors may develop lower limb lymphoedema after surgery, which negatively impacts quality of life. The purposes of this study were (1) to assess the levels of symptom distress, depression, body image, and health-related quality of life (HRQoL); (2) to recognize factors associated with HRQoL related in gynaecologic cancer survivors with lower limb lymphoedema. Methods A cross-sectional study was conducted with convenience sampling of gynaecologic cancer survivors with lower limb lymphoedema. Gynaecologic cancer survivors were assessed for symptom distress, depression, body image, and HRQoL. Multiple regression analysis was conducted to recognize the factors associated with HRQoL. Independent-samples t-test was used to compare symptom distress, depression, body image, and HRQoL by grade of lymphoedema. Results The most common distressing symptoms of lower limb lymphoedema were lower extremity oedema, lower extremity tightness, and lower extremity stiffness. Worse HRQoL was associated with more symptom distress, less satisfaction with body image, a high grade of lymphoedema, and a longer duration of lower limb lymphoedema. These factors explained 76.5% of the variance in HRQoL. Gynaecologic cancer survivors with late grade lymphoedema experienced lower HRQoL and higher levels of symptom distress, depression, and greater dissatisfaction with body image than those who had early grade lymphoedema. Conclusions Symptom distress had the strongest association with overall HRQoL and with all individual domains of HRQoL, except mental function. These results suggest that educating gynaecologic cancer survivors to assess lower limb lymphoedema-related problems, providing symptom management, and guiding survivors in physical activity to relieve lower extremity discomfort can improve HRQoL.

Background.-Lymphedema is a chronic and debilitating condition that affects many cancer survivors. Patient-reported outcome measures (PROMs) can give valuable insight into the impact of lymphedema on a patient's quality of life and can play an essential role in treatment decisions. This study aims to (1) identify PROMs used to assess health-related quality of life (HRQoL) in patients with lymphedema; and (2) assess the quality of the lymphedema-specific PROMs.Methods.-We performed a systematic search to identify articles on lymphedema, quality of life, and PROMs. An overview was created of all PROMs used to assess HRQoL in patients with lymphedema. The methodological quality of the lymphedema-specific PROMs was assessed using the Consensus-based Standards for the Selection of Health Measurement Instruments (COSMIN) criteria.Results.-A total of 235 articles met the inclusion criteria, of which 200 described studies using one or more PROMs as an outcome measure in patients with lymphedema. The other 35 studies described the development and/or validation of a lymphedema-specific PROM. The COSMIN assessment demonstrated that none of these PROMs met all quality standards for development.Conclusion.-The use of PROMs in lymphedema is increasing; however, based on our findings, we cannot fully support the use of any of the existing instruments. A well-developed lymphedemaspecific PROM, based on patient input, is needed to gain better insight into the impact of this condition, and can be used to measure the effect of possible medical and surgical treatments.DISCLOSURES Anne Klassen and Andrea Pusic are co-developers of the Q-PROM portfolio (including the BREAST-Q).
