Two-thirds of people aged ≥65 years have multi-morbidity. Multi-morbidity includes both physical and mental health conditions, as anxiety and depression, that almost invariably affect patients with multiple symptomatic chronic diseases. Management of a patient with any of the chronic diseases that are part of multi-morbidity is not just the management of that single index disease, but must include the active search and proper treatment of concomitant chronic diseases. The presence of concomitant chronic diseases should not alter the management of the index disease (e.g., chronic obstructive pulmonary disease [COPD]), and concomitant chronic diseases should be treated according to single diseases guidelines regardless of the presence of the index disease, obviously with careful consideration that this choice implies complex management, polypharmacy and potential adverse effects. Preliminary management programs suggest that a holistic integrated approach might improve quality of life and reduce hospital admissions and death in these multimorbid patients. Effects of hatha yoga on self-reported health outcomes in a randomized controlled trial of patients with obstructive pulmonary disorders

Yoga is gaining popularity as an alternative treatment but knowledge of its effects remains limited, particularly among patients with chronic conditions. This randomized controlled pilot-study investigated immediate and long-term effects of a hatha yogic exercises (YE) program as compared to a conventional training program (CTP) on healthrelated outcomes including anxiety, depression, stress, sleep quality, insomnia, and subjective health complaints (SHC). Patients with obstructive pulmonary disorders (N ¼ 36) were randomized into YE (n ¼ 19) or CTP (n ¼ 17). The YE included a newly developed 12-week program, adapted to the patient group, with two weekly classes delivered by experienced certified instructors. CTP training involved an individually tailored CTP. Questionnaire data were collected at baseline, at 12 weeks, and at a 6-month follow-up. ANOVAs comparing YE and CTP showed statistically significant interaction effects for anxiety and stress. In both groups, effects on anxiety were weak. Decreased stress was found in the CTP only. Separate analyses of each intervention showed consistently and increasing sleep quality in the YE-group between baseline and the 6-month follow-up, and decreasing SHC between baseline and the 12-week followup. For CTP, statistically significant effects emerged for both stress and SHC between baseline and the two follow-ups. Overall, comparisons showed more consistent effects of CTP, but for stress only. The findings suggest that YE and CTP effect different outcomes. This points at the importance of deciding which outcome to target when choosing between treatment alternatives.Abbreviations: YE: hatha yogic exercises; FEV 1 : forced expiratory volume in one second

Rates of anxiety symptoms in patients with COPD range from 13% to 51%, which is higher than rates for patients with other chronic diseases, yet evidence underpinning the effectiveness of the various treatment options is lacking. The aim of this review was to assess the efficacy and safety of both pharmacological and psychological interventions for the management of anxiety in COPD.We searched two Cochrane Specialised Registers of Cochrane Airways group and Cochrane Collaboration of Depression, Anxiety and Neurosis with complementary screening of Medline, PsycINFO and CENTRAL. Reference lists of included studies and online clinical trial registries were also explored. Randomised controlled trials (RCTs) and cross-over studies of pharmacological or psychological interventions for patients (age >40 years) with diagnosed COPD and co-existing anxiety (confirmed by recognised diagnostic criteria or validated measurement scale) were identified for inclusion. Data was extracted by two independent review authors with meta-analyses of outcomes performed using the random-effect model using Review Manager Version 5.3.A total of 1,792 citations resulted in nine included studies (five pharmacological and four psychological). Meta-analysis of these studies revealed significant reductions in anxiety symptoms, both with and without adjusting for heterogeneity, in the treatment arm (mean difference -2.84; 95%CI - REVIEW ARTICLE Trial registration number: PROSPERO: 2017:CRD42017056172Word count: 3105 (including Abstract, excluding references and appendices)
