ResumoRealizaram-se dois estudos com uma amostra de 208 participantes, pertencentes a uma Associação, com Doença Inflamatória Intestinal: 61% com Doença de Crohn e 39% com Colite Ulcerosa, maioritariamente mulheres (63,9%) e com média de idade 36,7 anos. Avaliaram-se as variáveis independentes em estudo: ansiedade, depressão, stress percebido, alexitimia e personalidade tipo D. A escala de personalidade não estava adaptada para português; no estudo 1, procedeu-se à replicação das análises psicométricas do estudo original. No estudo 2 determinou-se a predição das duas componentes da variável dependente "qualidade de vida". Através de regressões múltiplas hierárquicas, os resultados indicaram que, controlando a alexitimia, depressão e ansiedade são as variáveis com maior contribuição na predição da componente física. Na componente mental, controlando alexitimia e personalidade tipo D, observou-se que a depressão, ansiedade, perceção de stress e idade são preditores significativos. A personalidade tipo D tem maior prevalência nesta patologia do que em populações saudáveis. Palavras-chave: doença inflamatória intestinal, qualidade de vida, personalidade tipo D, alexitimia AbstractTwo studies were performed with a sample of 208 participants, belonging to an Association, with Inflammatory Bowel Disease: 61% with Crohn's Disease and 39% Ulcerative Colitis, mostly women (63,9%) and with 36,7 age mean. The independent variables in studies were evaluated: anxiety, depression, perceived stress, alexithymia and type D personality. The personality scale was not adapted to Portuguese; in study 1, psychometric analyses were performed according to the original study. In study 2 the prediction of the two components of the dependent variable "quality of life" was performed. Through multiple hierarchical regressions, the results have shown that, controlling alexithymia, depression and anxiety are the variables with the highest contribution in predicting the physical component. In the mental component, controlling alexithymia and type D personality, it's observed that depression, anxiety, perceived stress and age are significant predictors. Ulcerative colitis and Crohn's disease, commonly known as inflammatory bowel disease (IBD), draw attention from specialists of various disorders, including gastroenterology, psychiatry, and radiology. The involvement of a cortical influence in the brain-gut axis as well as the interaction of the hypothalamic-pituitary-adrenal axis and the peripheral nervous system provide an initial explanation of the psychological symptoms associated with IBD. The involvement of structures the limbic system, such as the anterior cingulate cortex, the prefrontal cortex, and the amygdala, paves the way for the discovery of the mechanisms underlying depression depression, anxiety, alexithymia, personality traits, and other psychological impairments following the onset of IBD. Psychiatric therapy in IBD patients is almost as important as the gastroenterological approach and consists of pharmacological treatment and psychotherapy. Neither of the available psychiatric treatment methods is considered the golden standard because both methods have side effects, and psychotropic medication can provoke the worsening of IBD symptoms. Thus, both approaches must be applied with awareness of the possibility of side effects. We suggest that psychiatrists and gastroenterologists work together to reach a consensus on IBD therapy to ensure success and to reduce side effects and relapse to the lowest possible rates.© 2014 Baishideng Publishing Group Co., Limited. Objective. This study thus aims to test differences between patients with inflammatory bowel disease (IBD) regarding IBD symptomatology, cognitive fusion, and psychological and physical health, as well as to explore whether the maladaptive emotion regulation process of cognitive fusion longitudinally impacts on the baseline and evolution of these outcomes over a period of 18 months.Design and methods. Participants include 116 IBD patients with a mean age of 36.76 (SD = 11.39) of both genders (69.83% females) that completed the self-report measures of interest in three different times, equally spaced 9 months apart, over a period of 18 months. Latent growth curve models were conducted using structural equation modelling to estimate the growth trajectory of the variables in study.Results. Inflammatory bowel disease symptomatology and cognitive fusion's levels were negatively associated with psychological health and physical health's baseline levels. Furthermore, IBD symptomatology did not influence the growth of psychological health, while cognitive fusion did (b = .30, p = .007). The same result was found for physical health (b = .26, p = .024). These findings indicate that individuals with higher levels of cognitive fusion present lower levels of psychological health and physical health that tend to further decrease over the time through the effects of this maladaptive emotion regulation process.Conclusions. This study implies that it is of crucial importance to include psychotherapeutic interventions in the health care of patients with IBD. If successful, these interventions could represent decreases in the cost of IBD treatment and in the use of drugs with adverse side effects, in addition to improving patients' mental health and quality of life. Further implications for clinical and research work are discussed. Statement of contributionWhat is already known on this subject?Research has demonstrated the impact of emotion regulation on both physical and mental health. Nevertheless, the longitudinal effects of the specific emotion regulation process of cognitive fusion on physical and psychological health have never been explored.
