The inclusion of caregivers in a holistic care approach represents a basic principle in palliative care. However, many palliative care professionals have a lack of understanding of difficulties or unmet needs among caregivers. To enhance the quality of life of caregivers and the quality of care for patients, healthcare professionals should be better informed about the constructs of caregiver burden. The aim of this study is to synthesize the concept of caregiver burden in palliative care, providing implications for the caregivers and their support systems. This concept analysis study adopts the integrative review approach and the basic text analysis method (ie, word frequency). The PubMed, CINAHL, Embase, and PsycINFO databases are explored for eligible studies.From this literature search, 66 articles from 1998 to 2018 are located. After data collection is completed, the two authors independently evaluate the quality of studies published before 1 September 2018. The caregiver burden is then redefined with its attributes, antecedents, consequences, empirical referents, and facilitators. It is recommended that the multidimensional concept of caregiver burden in palliative care be measured by considering caregiver characteristics and the caregiving context. K E Y W O R D S caregiver burden, concept analysis, integrative review, palliative care, word frequency 1 | BACKGROUND Developments in medical technology and the management of illness have increased the prognosis of terminally ill patients. As a result, caregivers have experienced the increased burden of caring for family members for an extended period of time. 1 Due to the decreasing number of days in hospital stays and the lack of financial support for home care services in palliative care, family caregivers are regarded as hidden patients or second-order patients. 2 Previous studies have reported that family caregivers experience a considerable burden from caregiving, and they become severely depressed and socially isolated from their communities. 3-5 For example, Grunfeld et al 6 and Grant et al 7 identified caregiver burden as one of the factors that most influence anxiety and depression in caregivers and more caregivers suffer from higher levels of perceived burden at the patientsʼ end-of-life than at the start of the palliative care according to their longitudinal studies. This increasing caregiver burden in palliative care can be explained by patientsʼ declining health status, increasing demands for patientsʼ symptom management, and the need for decisions regarding terminal care (ie, enteral tube feeding, advance care planning, and do-not-resuscitate). Nevertheless, numerous family caregivers do not perceive their burden as problematic. De Korte-Verhoef et al, 8 found that even though 66% of family caregivers experienced a severe burden from caregiving, only 25% indicated that their burden had a negative impact on their daily lives. Este estudo objetiva verificar se ocorre o sentimento de vulnerabilidade ou não e se o cuidador percebe empatia e apoio por parte da equipe assistencial. Trata-se de um estudo descritivo e qualitativo. Os participantes foram os cuidadores/familiares de crianças e adolescentes internadas num hospital pediátrico da região do Vale do Itajaí. A técnica de coleta de dados foi a entrevista semiestruturada. Os dados foram analisados por meio da análise de conteúdo temática de Bardin. Participaram da pesquisa um total de 11 cuidadores/familiares, sendo todas do sexo feminino. Em relação ao parentesco, 09 eram mães, 01 irmã e 01 era tia. As três principais categorias de investigação que emergiram foram: motivação– melhora na Família, sentimentos dos cuidadores/familiares durante o processo de internação e sentimentos dos cuidadores/familiares durante a visita dos profissionais da saúde. O cotidiano do cuidador/familiar é caracterizado por processos que visam um objetivo único, que é a recuperação da saúde da criança e adolescente.

Palliative care is an integrated approach that promotes the quality of life of patients and families who are confronted with the physical, psychosocial and spiritual problems associated with a life-threatening illness. 1 The palliative patient is not necessarily a terminal patient (estimated prognosis of 3-6 months), or a moribund or agonising patient (estimated prognosis of days or hours). 1 According to the World Health Organization (WHO) and the National Plan of Palliative Care, palliative care: affi rms life and accepts death as a natural process, without trying to delay the process; constitutes a global and holistic approach to the patients suffering from a physical, psychological, social and spiritual point of view; seeks the wellbeing and quality of life of the patient; is centered on the needs of patients and their families, life and death; should not be restricted to terminally ill and agonised patients; and, should be addressed by a multidisciplinary team. 1,2 The person with a life-threatening illness feels in total pain. 1 Patients with an advanced life-limiting illness experience great suffering, a complex negative state of malaise (bodily, affective, cognitive, spiritual) characterised by feeling threatened and destroyed in the integrity or continuity of their existence, a sensation of powerlessness to face this threat and an exhaustion of their personal and psychosocial resources. 1 The main sources of suffering in palliative care patients are poorly controlled symptoms, loss of autonomy and dependence on others, alterations in body image, loss of social roles and status, impairment in interpersonal relationships, feelings of abandonment, changes in expectations and future plans, and loss of dignity and meaning of life. 1 In most cases, palliative patients have severe functional and cognitive limitations requiring support in basic needs, such as hygiene, food, money, medication and mobility, relying on others for daily life activities, with increasing dysfunctionality and psychological repercussions. 2 According to number 7 of the Administrative Rule 66/2018 in Diário da República (Portugal), 3 a palliative care unit must ensure: permanent medical and nursing care; psychological intervention for patients, relatives and professionals; social intervention and support; support and intervention in mourning; spiritual intervention; complementary examinations; prescription and administration of drugs contained in the National Formulary of Medicines; hygiene, comfort and food; conviviality and leisure; training in palliative care; and, assistance in the area of palliative care for health professionals, namely primary, hospital and continuing healthcare. 3 Psychosocial rehabilitation can be understood as, 'a comprehensive and continuous process that provides individuals with disability due to mental illness the opportunity to achieve the greatest possible functioning potential (in the Palliative care is an approach to incurable and/or severe disease with limited prognosis, aiming to relieve the suffering a...
