Although many adolescent risk assessment tools include an emphasis on dynamic factors, little research has examined the extent to which these tools are capable of measuring change. In this article, we outline a framework to evaluate a tool’s capacity to measure change. This framework includes the following: (a) measurement error and reliable change, and (b) sensitivity (i.e., internal, external, and relative sensitivity). We then used this framework to evaluate the Structured Assessment of Violence Risk in Youth (SAVRY) and Youth Level of Service/Case Management Inventory (YLS/CMI). Research assistants conducted 509 risk assessments with 146 adolescents on probation (101 male, 45 female), who were assessed every 3 months over a 1-year period. Internal sensitivity (i.e., change over time) was partially supported in that a modest proportion of youth showed reliable changes over the 3-, 6-, and 12-month follow-ups. External sensitivity (i.e., the association between change scores and reoffending) was also partially supported. In particular, 22% of the associations between change scores and any and violent reoffending were significant at a 6-month follow-up. However, only 1 change score (i.e., peer associations) remained significant after the Bonferroni correction was applied. Finally, relative sensitivity was not supported, as the SAVRY and YLS/CMI was not more dynamic than the Psychopathy Checklist: Youth Version (PCL:YV). Specifically, the 1-year rank-order stability coefficients for the SAVRY, YLS/CMI, and PCL:YV Total Scores were .78, .75, and .76, respectively. Although the SAVRY and YLS/CMI hold promise, further efforts may help to enhance sensitivity to short-term changes in risk.

The purpose of this study was to explore the effects of alliance dimensions and involuntary versus voluntary referral status on the therapy outcomes in family therapy. The first and fourth sessions of 29 therapeutic processes with involuntary and voluntary families seen in family brief therapy were rated with the System for Observing Family Therapy Alliances . This instrument evaluates four dimensions of the therapeutic alliance (Engagement in the Therapeutic Process, Emotional Connection with the Therapist, Safety within the Therapeutic System, and Shared Sense of Purpose within the Family). The outcomes were measured with the Goal Attainment Scaling (Kiresuk, Smith, & Cardillo, 1994). The obtained results highlight two main conclusions: (a) There were no differences in the outcomes between involuntary and voluntary families and (b) in the fourth session only, there was a significant effect of "Safety within the therapeutic system" and "Shared purpose within the family" on the outcomes, namely, those alliance dimensions that are unique for conjoint therapy. Important clinical implications are discussed concerning the therapeutic work with involuntary families.

The purpose of this study was to examine the association between characteristics of the symptomatology change curve (i.e., initial symptomatology, rate of change, and curvature) and final treatment outcome using a multilevel approach to account for therapist effects. The sample consisted of community clients (N = 492) seen by 204 student therapists at a training clinic. Linear, quadratic, and cubic trajectories of anxiety and depression symptomatology, as assessed by the Shorter Psychotherapy and Counselling Evaluation (sPaCE; Halstead, Leach, & Rust, 2007) were estimated. The multilevel quadratic trajectory best fit the data and depicted a descending curve (partial U-shaped). The quadratic growth parameters (intercept, slope, quadratic) were used as predictors of final treatment outcome as assessed by the Outcome Questionnaire-45.2 (OQ-45.2; Lambert et al., 1996). Patterns for two different successful outcomes, as conceptualised by (a) change in general symptomatology and (b) reliable improvement, were identified. For symptom change, successful outcomes followed a pattern of low initial levels of depression and anxiety, high initial rates of change, and high (flattening after initial drop) curvature, and the pattern applied to both within- and between-therapist levels. For reliable improvement at within-therapist level, successful outcomes followed a pattern of high initial rate of change and high curvature. For reliable improvement at between-therapist level, successful outcomes were associated with a pattern of low initial levels of depression and anxiety. Implications for clinical practice are discussed.
