Introduction: Geriatric population is growing faster than all younger age groups. More than 20% of the geriatric people suffer from a mental or neurological disorder. Various studies have been conducted in Nepal related to mental and neurological disorders in geriatric people including epidemiological survey, outpatient department (OPD) based studies and inpatient studies. The aim of the present study was to determine the prevalence and nature of psychiatric disorders in geriatric subjects Material And Method: Geriatric subjects attending outpatient department of psychiatry, Nepalgunj Medical College Teaching Hospital (NGMCTH), Kohalpur, Nepal from January to August 2018 (8 months) were enrolled for the study. Ethical clearance was obtained from the Institutional Review Committee of the NGMCTH and written consent was taken from the subjects or next to kin. Socio-demographic profile and other details of the subjects were obtained. Diagnosis was made using the ICD 10. Data was analyzed using Statistical Package for the Social Sciences (SPSS) version 21.0 for Windows. Results: The prevalence of psychiatric disorders in geriatric subjects was 6.25%. The mean age of the subjects was 68.6 ± 7.39 years. Female constituted 51.1% of the subjects while male 48.9%. Category wise, mood disorders was the most common diagnosis (44.9%) followed by organic disorders (20.2%) and neurotic disorders (12.9%) respectively. Mood disorders and organic disorders were significantly associated with the age of the subjects (p<0.05). Disorder wise, depression was the most common diagnosis (39.9%) followed by dementia (12.4%) and anxiety disorder (10.1%) respectively. Dementia and delirium were the only disorders significantly associated with the age of the subject (p=0.01). There was no statistically significant association of individual disorders with sex of the subjects. Conclusion: Depression, dementia and anxiety disorder was the most common diagnosis in geriatric people.

ObjectiveThis study was conducted to comprehensively understand the context, barriers, and opportunities for improving dementia care, treatment, and support. The objective is to guide the development of a national dementia care plan.MethodologyThis document review was conducted by analyzing literature available in the public domain, including scientific publications, project documents/reports, media reports, and hospital records. Additionally, annual reports published by the Department of Health Services, national census and demographic and health survey reports, Old Age Homes, and other relevant government reports were examined. Firsthand information was gathered from relevant stakeholders based on the World Health Organization's situational analysis framework for dementia plans. This framework encompasses four domains: Policy context (national ministries, legislation, policies, strategies, plans related to dementia, mental health, aging, and disability), service delivery assessment (health and social care workforces, services, support and treatment programmes, and promotion of awareness and understanding), and epidemiological indicators (prevalence and incidence rates of dementia, risk factors). Ethical clearance was obtained from the Institutional Review Committee (IRC) of B.P. Koirala Institute of Health Sciences (IRC no.2658/023).ResultsExisting policies in Nepal inadequately address the needs of people with dementia and their caregivers. Concerning health services, the Government of Nepal provides financial subsidies to individuals diagnosed with dementia; however, numerous hurdles impede access to care. These obstacles include geographical and structural barriers, an inefficient public healthcare system, weak governance, financial constraints, low awareness levels, stigma, and inadequate workforce. Furthermore, the absence of robust nationally representative epidemiological studies on dementia in Nepal hampers the development of evidence‐based plans and policies. Similarly, there are no interventions targeted at caregivers of people with dementia, and no initiatives for dementia prevention are in place.ConclusionsThis review underscores the urgent need to formulate a comprehensive national dementia care plan to address the growing challenges. Key priority action areas include the integration of dementia care into primary healthcare services, training workforce to provide the care, increasing awareness, mitigating stigma, developing caregiver support programs, and initiating high‐quality research to inform evidence‐based policymaking.

Psycho-social problems among older people residing in community of Chitwan, Nepal—A cross-sectional study

Background Older people are vulnerable to various psycho-social problems such as depression, anxiety, insomnia, dementia, and loneliness that have profound impact on overall health and their quality of life and higher use of health services. Prevention and treatment of psychological problems in this risk group is critical for improving their quality of life. Objective This study aimed to find out the psycho-social problems among older people residing in community. Methods The study was a cross-sectional study design and 388 older people aged 65 years and above residing in different wards of Bharatpur Metropolitan city were selected using probability simple random sampling technique. Data were collected using interview schedule containing geriatric depression scale-15; Geriatric anxiety scale-10, University of California Loss Angels Loneliness Scale (UCLA-20), Anthens Insomnia Scale and Mini-Cog Test. Obtained data were analyzed in SPSS version 20 for windows. Chi-square test was applied to measure the association between psychosocial problems and selected variables. Result Findings of the study revealed that the mean age (± SD) of respondents was 72.92 (±7.12) years. Almost all (93.6%) older people had full functioning of activity of daily living and two third (66.5%) had other co-morbid conditions. More than two third (67.0%) of older adults had depression, 60.3% had anxiety, 53.6% had moderate to high level loneliness, 47.2% had insomnia, and 33.3% had dementia. Age, functional dependency, sex, co-morbidity, financial dependence, education and occupation were significantly associated with the psychosocial problems among older people. Conclusion and recommendation Psychosocial problems are common among older people residing in community of Chitwan. Hence, there is need to develop and implement health care strategy by local health care planner to prevent, treat and manage the psychosocial problems among this risk groups. Further, health care providers working in geriatric problems or psychosocial health need to conduct regular screening programs for the early diagnosis and treatment of these problems.
