PSMA-PET/CT-Positive Paget Disease in a Patient with Newly Diagnosed Prostate Cancer: Imaging and Bone Biopsy Findings

Abstract: A 67-year-old man diagnosed with Gleason score 4 + 5 = 9 clinically localized prostate cancer with 68Ga-labeled prostate-specific membrane antigen-targeted ligand positron emission tomography/computed tomography (PSMA-PET/CT) positive Paget bone disease is described. Immunohistochemical staining revealed weak PSMA positivity of the bone lesion supporting the hypothesis that neovasculature might explain positive PSMA-PET/CT findings in Paget disease.

“…However, PS-MA-avidity is mostly teardrop-or nodular-shapen in lymph node metastases in 50-70% of cases and only rarely (about 1%) in sympathetic trunk uptake (46). Also in benign bone pathologies, where there is a setting of increased vascularity, bone remodeling, and reparative processes like in Paget Syndrome or anemia (47).…”

“…The rapidly expanding role of 68 Ga-labeled PSMA PET/CT in prostate cancer needs careful evaluation as its specificity is limited by some false-positive findings as enumerated in the present article [ Table 1 ]. “…Subgroup analysis further supported the superiority of PSMA in all studied clinical settings -primary staging, secondary staging and those with metastatic castrate-resistant prostate cancer. Despite the exquisite sensitivity and specificity of PSMA -a true example of bench-to-bedside translation in molecular science nonspecificity of PSMA seems to arise within certain nonmalignant processes such as Pagets disease [16], as well as certain other cancers including hepatocellular carcinoma, clear-cell kidney cancer and brain tumors [17].…”
